Every single time
- Changing quantity is almost 100% safe. You can set quantity to zero(very bad idea), but why would you?
- The transformation from one item to another - a bit dangerous.
- Creating items - very dangerous. Also created items don't have attached script. (e.g. Fallout 2 Hintbook won't work)

back in the days I used a tool called FallEd. But it was more like a Trainer than an advanced Editor (it could transform specific items that you could get for starting the game with certain levels of certain Skills into other items. Like, transforming Stimpacks into a Plasma Rifle, Plasma Rifle to Turbo Plasma Rifle, Turbo Placma Rifle back into Stimpacks, Lockpicks into Electronic Lockpick, Medkit into Combat Armor and so on, or something like that, and i think it also could boost all your SPECIAL stats to 10, and some other options.)
I can't find it online right now, but I think I should have it burned to a CD-R disk, along with some other very old Fallout-related files (mods, saves, etc.)
