So where's the worm boss?

I assume it has to be in the caves since it's the only zone I've been through with no associated boss fight (as well as the only zone with worms) but I can't find it anywhere

[Spoiler] Some Achievement don't trigger

In total 3 achievements do not trigger - Dark beast - Dark Beast Full Strength - True Ending - I defeated the boss once with a bad ending => I guess should trigger the normal Dark Beast achievement. - I defeated the boss once with the good ending (6 guys helped me) at Day XX - I defeated the boss once with the good ending (6 guys helped me) at Day XX and with the "???" guy boosting all enemies (boss had around 24k HP) But none of the 3 achievements triggered.
