I had a bug that crashed the game upon death with this structure built. I wish I'd kept the error for you. However, when I relaunched the game my save had been respawned so it kind of worked.

Resurrection shelter never worked for me : after I died, it seems I'm going to respawn and then the game crashes. Once I relaunch the game, I see the shelter a few seconds before the game crashes again..

I have been using this mod for a while. It crashes the game upon death/respawn too. But usually when I relaunch the game the older autosave is loaded. So, as Nutty said, it kind of works - and it allows me to live forever if I so wish.
Then I died in the Caves. This time the game loads a save right before the respawn. And then it crashes, again and again. Similar to what Flux described above.
It happened in base DS. ROG version seems to work just fine.

I have a screenshot of the crash, too.
