I think it would be best if it were a Scream fortress item
This is because it kinda changes medic's body shape and proportions, and normal cosmetics probably shouldn't do that, I'd go more in depth of why I think this, but I think you understand.:demoticon:

if you worry about hitboxes don't worry. They added cosmetic that litreally shows medic's abs and it dosen't change his hitbox.

I'm not worried about hitboxes, it just is weird to me. And yeah, the other cosmetic I think should also be on the same boat as this one.
