there could be a small explosion right when he shows the painting so the enemy sees it.

this shouldnt have a taunt kill, its nice as it is. A well made item that I think would fit very well into the game. Nothing need to be added.

At least he's imitating a gun with his finger, which makes somewhat sense, while a random explosion upon showing a painting doesn't make sense.

What they could do for a taunt kill effect is the enemy is so suprised that they do a "Heavy pow" ragdoll effect

the heavy does not have a bullet it makes the enemy so scared they have a heartattack and that could be done with the painting, they are shocked at the fine art work they die
