I have quite a few suggestions bouncing around my head at the moment, so I'll drop them here:
* Sounds for the breathing that occurs after surfacing from low air, or after pushing too hard during starvation or as gourmand.
* Subtle sounds for the bubbles released by a bubble weed or creature low on air.
* Perhaps a noise for shivering while cold?

If visuals are being considered as well, here's one:
* When air is low, or gourmand/starvation exhaustion is kicking in, have a haze creep in. (Like the one with hunter on negative cycles, but grey instead of red, and no sound)

@Bray-G I'll try to add some of your suggestions tomorrow or maybe all

Played a bit of arenas with it. I'd say the mod doesn't change much, but it's really a welcome change, except for the times when the room ambiance starts to sound janky for whatever reason. especially explosions, those get crunchy sometimes. AM_SUB-CaveTone sounds really janky on Hole. I really like the reverb, think it's barely noticeable when using the mod but will be immediately missed. It sounds really good on levels tho, wonder what method you're using. Wind sound was also pretty nice.

Now for the actual suggestions.
- Deafness on stun should fade out slower instead of disapearing the moment you stop being stunned, current implementations chops the audio too quick.
- Make the sounds actually muffled instead of very quiet on death and stun.

you should rename the fear heartbeat to Adrenaline heartbeat

most of the slugcats would probaly not exactly be fearful when fighting a creature but just well, be in a state of adrenaline as they push themselves to kill the creature

specially because of the whole cycle thing i don't think any creature fears for their death as it just doesn't matter
making flashbangs actually give you blindness for a bit could be cool

Perhaps subtle breathing sounds when near a lizard?

Maybe The screen turning black when you die?

this would be disabled by default tho

oh yeah please add this, i found out on the wiki the slugcat is in fact affected by the blindness effect it just doesn't do anything
Idea:

effect for when being affected by starvation (heartbeat, muffled sounds, blurry)
