I can literally turn off every mod, but have just this one, and it runs slowly.

In my experience this mod doesn't cause significant lag and doesn't have a load of egregious code running that would cause it. You can run the lag check mod to see whats causing tons of ticks while you're playing, to figure out whats causing it.

You mean the thing from like 2018?

Yeah... you should probably not be using outdated API mods... They can screw things up bigtime.
