My guess is that this was done with an editor mod (i.e. bee mod) that allow for the placement of linked_portal_door entities, a Portal 2 specific entity which allows for the seamless transition and rendering between two places. They're similar to standard portals, but without the border and with configurable size. By default, they're only usable when using the Hammer map editor for Portal 2.

No idea if this is true or not, so take it with a grain of salt.

This is exactly how I did it! I used the world portal in the items-4-clean addon pack for BEEMOD.

Install BEE2.4, and then download and install the items-4-clean addon pack. This will give you access to the blue panels, which are linked-portal-door entities

I'd also be interested in this addon pack. I was also unable to find the one you mentioned.
I did find a pack that includes world portals but those did not seem to work properly.

They do work. I played around with the world portals for a bit and you have to obviously connect two of them to each other but you also have to right click both and check "start-enabled" for them to actually work.
Alternatively they can also be powered by things like buttons to make them work.

Thanks @ishouldhavedragons for providing a link :D
