Concerning the "smokey mess" - that scene also suprised me on my very first attempt, Emily got shot instantly and before you could see anything. So, what are you going to do when you know what will happen at this spot? Right, shoot into the smoke right when you hear the detonation. So indeed, this is some sort of "shoot faster", but not difficult at all and somewhat fool proof.
For a more general piece of advice on survival: Take cover, let them run to you, and watch your back. If your aim is as terrible as mine, just shoot like a mad man, reload frequently, and don't ever worry about wasted ammo or too much noise. You'll be the lousiest killer of all time, but the game mechanics do not punish you for acting like an amateur. As long as it works, it ain't stupid, right?
