can i check which version you are using? (if you can find the mod's dll, and do a file>properties it should show 1.2.4.0?

what happens if you try a quick test with a brand new game in vanilla
and if surgery works, if you then add just rimtrheaded?
it might be worth trying to unsubscribe from this mod, and to wait 5 mins and then exit and relaunch steam, and then to re-subscribe (just to make sure that the latest version has fully been reinstalled?)
(or you can manually zap the folder when steam is not on, and then resubscribe?)
