How does the Institute do it?
To match Kenshi itself and the Doll lore specifically, it is my intent to keep things vague as to how the Institute actually makes available your new companion(s).

I think my general theory was/is that The Institute finds and refurbishes existing Dolls. They need to inspect the player in order to learn enough to fix one of their currently-disabled Dolls. Lore-wise, the Institute has a secret location with a warehouse filled with deactivated Dolls, but this has not been implemented in the game. Maybe they stumbled upon it at some point.
