Incompatibility with EPOE-Forked: Royalty DLC expansion
When EPOE-Forked: Royalty DLC expansion is loaded with Elite Bionics Framework, Visual Exceptions report an error as:

The error doesn't appear in the debug log itself. It is only reported by Visual Exceptions.

It took me forever to figure out that this was causing my issues. My game would not even load to the main menu, and since it didn't give off any error for me.
