Cities won't grow
I really really like the idea but I have yet to get very far when playing this mod. My cities get stuck at 1 or 2 population while the AI civs achieve a much more reasonable 4-10 in the same time span. My cities claim they will increase in population in _ turns, but at the start of the next turn it always resets back to the same number. After nearly 100 turns without growth I couldn't handle it anymore.

There's no plague in my cities, I'm not building settlers, and I have high happiness and I haven't selected the 'avoid growth' option. I deactivated all other mods. I've tried numerous civs (king difficulty, starting in ancient era). I've even had population in the city freeze within the first 16 turns.

Is this intentional, a bug, or am I doing something wrong?

Not a bug at all. As the description clearly says, Health adds to city food stores each turn. So if your city's health is negative that will subtract food from your stores. So you either have to add to health or generate enough food to overcome the negative health to get your city to grow. Health is just like any other yield: if you want more of it you have to build improvements, buildings, etc. See in-game help for more information.

Probably negative health - should you start in flood plains, either walk around your settler, or reroll.

what is the minimum health number that stops city growth?
