When you put the sample under the stage beam fly out of the map and you will see a small room with buttons in it. Go into that room, and press your interaction key (mainly E) on the button labeled "Trigger Disaster"

idk all i know the lump file or the needed content is more important than some incident i suppose. isnt it?

it doesn't work how do I get the sample??

You have to go to the observation room and click the console on the wall next to the door down to the test chamber. i think you can trigger the cascade even without the sample, idk tho.
