Simple VTMB IK rig for very tall heels

Edited biped_simple for my VTMB ports, specialized for characters with very tall heels. No clue why the VTMB models have the clavicles attached to the neck, but this will move them to the spine.

biped_simple uses the offsets from the feet to the toes to calculate the offsets for the knee- and elbow controllers. On some VTMB characters, this will lead to the controllers being poorly positioned. This edit uses extra helper bones, instead of the toes, to calculate the offsets. It'll only work on models with these helper bones added.
