the latter issue is in fact SFM being SFM, it will always do this when models receive rig updates, you can usually fix this by right clicking on the affected models, going to "utilities", and clicking "reattach to model". as for the other issues, as far as i can tell this is unique to you
my recommendation (for this and all other issues with any SFM addon), are to unsubscribe from the pack, launch SFM and agree to remove the pack's files from your install, resubscribe and allow SFM to reinstall the addon contents, then test the model in a NEW session
as usual make sure to press "yes to all" when SFM asks about file conflicts
