It does work with Rep as well, althought some items names are redunant/sprites cause confusion.

As of Repentance there is an official Almond Milk. Can I suggest the name "Rice Milk"?

It does work with rep i recommend using soy & choccy milk reskins so you can tell wich one is wich betwwen the modded milk and the non modded but the only issue is WHY THERE ARE 2 FUCKING ALMOND MILK TYPES

Add a cow transformation. Your face becomes an udder which shoots milk instead of tears leaving milk creep that has effects based on every milk item you've picked up.

Almond milk gives you soymilk speed shots, slightly higher damage, but makes your shots go in random directions, all the items are listed on eluc's site Platinum God. (All Antibirth, Og items, Rebirth, Afterbirth, and Afterbirth+)
