Fits the character well, has that nice shade of grey-ish-black which fits the palette, lovely details with the sticker and the strap... A good solid item here, and definitely one I think Valve should look at! My only issue is that, like others have said, it seems to jut out with its size. Maybe you could try angling it so it's a little less horizontal?

I think there's already an animation in the game unused which depicts the engineer playing a long tune on his guitar and then putting it away instead of crashing it. You're idea is a bit too complex but I think it could be a hold-action item which makes the engie play the animation on loop along with the song until you let go of the action-button. TL;DR, VALVE, MAKE THIS HAPPEN!

pressing the action button makes him take it off, take out his guitar, sit down on the case, and play "more gun" until the song is done or you interrupt it with any action whatsoever (walking, jumping, shooting, etc.).
