The cape, cowl and pants are a single item.

The reason why the bandolier goes on the outside of the cape is because the bullets themselves clip with the Heavy while he runs. Once you have a cape that needs to always show above wherever the bullets are it means the cape then has to clip significantly into the Heavy model. This just looked bad so I went back and remade the cape to go underneath. The bottom of the cape still moves as you run but obviously the top half needs to be restricted.

The reason why this isn't available for multiple classes is because a black cape would obscure the team colouring too much. The majority of the Heavy's team colouring is at his shoulders, which I've intentionally made sure are still visible from behind. His jacket is a dark colour to start with so the addition of the cape doesn't obscure visibility from a distance. The only other class I think this could work with is the Demoman, and if I have time before the deadline I will try this out.

Confused, is this just a Cowl or is it the Cape and the thing on his legs too? If it's the cowl there's an all-class one already, a cape would be great though.
