The joke is:
There's a faction named "Soldiers sans Frontiers" (Soldiers without borders in EN) so they made the costume of an Soldiers sans Frontiers related character (of MGS) and they named it "Doctors without borders" because of that Organization.

And the Description of the addon is an 3rd joke, so there's so much jokes here that made this the addon of 2015!
