Unfortunately, yes. Both mods have to edit the files that make up the entire UI for civs, images are stored as frames in an SLP file; a mod that edits an SLP has to overwrite the entire file. Not a lot I can do about that, I'm afraid :(

It should be working for all civs except those in African Kingdoms; I need to update to include those. Again, check it's in the in-game mod manager and that it's installed properly.
No, you can play the game just fine without it. It's just a matter of choice, really. To some people the old icons just match the feel of the game better. I'm happy playing with either, though I do have a soft spot for the old ones! It's not a massive change, it's just there to give a choice really. No harm in that :)
