It's very rare for a player to be take enough hits during a floor to reach maximum insanity for obvious reasons, and if that's actually the case, it's almost certainly because of Blank Card + Holy card that they were able to take that many hits, meaning that they likely aren't actually losing their sanity and they feel fairly secure. Even with the combination though, they likely won't take that many hits throughout the floor.
