Whenever you get given a blacklisted Pill by Mom's Bottle of Pills or a Caffeine Pill - or get given a blacklisted Card by Deck of Cards - the pocket item will now be replaced directly rather than by forcing the player to drop it.

Refactors the pickup replacement function to use the ~Brand Spanking New~ MC_POST_PICKUP_INIT mod callback instead of MC_POST_UPDATE.

As well as being slightly more efficient, this should remove the very brief delay in blacklisted collectibles being replaced when you enter a room.
