Official TF2 Cosmetics can be found in the ''Games'' section in the spawnlist this way

The one you are looking fore can be found like this:
Games>Workshop>Player>Items>Medic>Surgical_stare
(Click the one the folder named ''Surgical_Stare'')

The icon will appear as tiny in the upper left corner of the spawnlist. click on it to spawn the mask.

Also it turns out the cosmetic was originally named ''Surgical Stare'' on the TF2 Workshop, and the folder retained its old name instead of what Valve renamed it.

Thats because you are supposed to bonemerge it to the TF2 Ragdolls with The Easy/Advanced Bonemerge Tool, not put it onto your playermodel.

Its an addon made for posing purposes(Videos, Artwork, Etc.)
