How it all works
The available rewards are grouped in three tiers.
  • Dying before floor 6 gives you tier 1 (i.e. nothing)
  • Dying on and after floor 6 -excluding the last floor- gives you tiers 1 and 2
  • Dying on the final floor gives you tiers 1, 2 and 3
Also:
  • Whether a floor has enemies, shrines or NPCs makes no difference.
  • The last floor of a dungeon will give you tier 3 even if the dungeon is less than 6 floors deep (as is the case with the first dungeon).

After you finish the game you gain access to an infinite dungeon. Since there is no final floor in this one, tier 3 will not be available.

Resurrects player

Notes about resurrection:
  • You are resurrected one level before the one you died in.
  • Everything that has happened in the dungeon in you previous run stays the same.
This includes: killed enemies, bought merchant inventory, received blessings and buffs (except those that 'expire in x seconds'), previous dungeon rewards, moved items between chests, used items, accumulated gold, and current inventory.
Getting them all
The easiest way to encounter all of the rewards and obtain their descriptions in-game, is to delve into the first dungeon: only 4 floors deep, with very easy encounters.
Just keep dying on floor 4 until you get them all.
Happy hunting!

I was wondering about this! I could never get the one with the red sword no matter how many times I died. lol. Pretty unlucky.
