To speed up finding them, I suggest to complete the 4th Pirate's quest "The Curse of Jah" - you'll receive the Artifact "The Dead Man's Compass", which shows them on your map once you're nearby.

Note: Only 48/50 eyepatch. The achievement still triggered in another playthrough.The remaining 2 either:

should have been in Puerto Cabezas, but aren't.
I missed them. In that case I'll gladly include them if someone can point me towards them.

Basseterre - 2
- south of landing point
- next to bank

Belize -1
- top right corner, near a wodden bridge

Buccaneer Stronghold - 2
- north of the unit's building, near entrance to beach
- directly south of the Alchemist, across the street hidden by two palms

Cancun - 3
- bottom right, from the bank scroll up about 4 screens, next to the big destructible building
- top left corner at the beach behind the 2 big towers
- top left corner, about 1 screen to the left of the 2 big towers

Caracas - 1
- top left corner, near building on beach

Colon - 2
- follow the shore on your right upward, next to a big firepit
- north of the landing point, next to a small lake in the center

Corsair Stronghold - 2
- next to the landing point
- next to Alchemist

Eagle Island - 2
- north of landing point, on the left next to the first hut, before entering town
- top right corner, next to a tower, about 2 screens to the right from the bank

George Town - 2
- to the right along the beach, then north, on the right side of town
- from landing point northwest, about 2 screens below the bank

- top left of map, about 1 screen to the right of the bank
- top right of map, a bit northwest of the two big towers

- stick to the left, all the way up until you reach the town area

Note: You either picked Slowdown Harpoon, Human Catapult or Mine Barrels at the start of the game.

Having spent some time searching for the last eye patch I can also confirm the achievement pops on a second play through.

So I know this is an old thread by now, but going through this game after a long period, I thought I'd try and fill in a few gaps :) I'm still searching but confirm 1 of the 2 missing eyepatches in is 'Manzanilla' (for a total of 3) it's in the centre of the map slightly north of a small fountain, just to the west of the fields
