How do we arm first?
Get pipewrench
It's can be found in first door on first stage
or in cabinet of GP on second stage.
Looking for really big gun
Get shotgun. It's located on third stage in restaurant
Kids playng with guns
Get pistol. It's located on second stage at office.
Did I play video games because I was miserable?
Get flashlight. It's located in second room at first stage of real ending.
The fourth dimenssion wall
Collect Dr.Randel newsclips. First located at second stage.Second one located at restaurant on third stage.
Third clip located on fourth stage.
Bonus: all codes
First door code - 437, second door code - 209, case code - 929

Seems like it was a bug after all. With the 2.5.0 update, the third clipping reappears at its place. Mind though, you need to do a fresh run through the game.

Same as erc, can't find the third newspaper anywhere.

Can't find the third clip at the shown point (or any other place for that matter) in the fourth stage. Any ideas?

Short and sweet, thanks for the guide! Just curious though how you got the second code? I couldn't figure it out...
