FPS/Arrow Key Style
After some experimentation I have discovered that these "FPS and arrow key" controls work best for me:

Control
Key
Grapple
Shift
Upgrade
S
Exit Vehicle
Space
Left Car
A
Middle Car
W
Right Car
D
Movement
Arrows

With this setup you should be able to set your left hand like you are playing a normal FPS game and your right hand on the arrow keys.
Less Clunky Arcade Style
Here is a style I have found to be less straining on the hand but it still allows for the "zxc and arrow key" controls. It isn't a huge change but I found it to make a huge difference.

With this setup you should be able to set your left hand like you are playing an arcade game but wont have to strain to use grapple and your right hand on the arrow keys.

The game has joystick support, which is amazing.
I bought one out of curiosity and it works great. It has 3 side buttons for switching cars, a trigger for horn (obviously), tiny buttons for messing with the upgrades and a nice curved side button for jumping out of trains going 250km/s. It also has a smooth lever, which probably only works as a binary button in most games, that I put to apply upgrades (I feel like I'm sending a nuke every time I put on the helmet). It's dumb as hell, but damn worth the 20 bucks!
Too bad it doesn't seem to work on a lot of games, probably only the ones with joystick controls? I don't see why I cant just change inputs to whatever Wii Fitness Board I'm using.
