Introduction
Hopefully this guide will help you unlock the achievements you are missing in the game. As I was playing, I had a little trouble with a few where they were not fully outlined in the details, whilst others just required a great deal of grinding, that could have been done more efficiently.

I will not be telling you how to play or what choice of characters to take. I think they are all viable and depend on your playing style. What worked for me may not work for anyone else, but I will offer my recommendations based on what I did as a rough idea.

Do not expect to get all of these acheievements in a single playthrough. Whilst not impossible, it is just not really worth running around the same areas repeatedly, as it is counter-productive to getting other acheivements, so be prepared to go through the game at least twice.

I should also note that this based upon my in game experience. I have not looked into code or combed through forums. As such, my observations may be incorrect. If this is the case, please leave a comment and I will gladly adjust the guide.
Main Story & Quests
Welcome To The Dead World
Urban Decay
Parks And Desecration
Residential Evil
Dock & Cover

In order to unlock these achievements, you only need to follow the main quest lines, and one will unlock with each chapter of the story. Just follow the missions in blue. There is no need to do side quests or go any further in the dungeons than necessary, though I would recommend doing this to get the other achievements and the drops make these 5 achievements easier to finish.

Quest Love
Just keep following objectives and you are guaranteed this achievement.
Travel & Dungeons
Explorer
Archaelogist (Not my spelling!)
Dora

These three will unlock just by making sure you do side quests. They will lead you to the necessary 25 locations.

Faster & Furiouser
Easily done. Just use a bus-stop to move from location A to location B. You will be using these loads so do not worry about getting this achievement.

Diver
I recommend you do this in the Jupiter Enterprises Building, just before you head into the sewers. Your characters will have enough experience to handle it, and doing it here helps you unlock other achievements also. There is a fairly brutal fight on the 20th floor, but just remember that signal flares are your friend and you will be fine.

Chester Copperpot
If you are doing the dungeons in order you will unlock this on floor 10 of the caves. Just keep grinding.

Henry Jones Jr
Now this is an easy one to miss, but simple enough to do. You just need to grind it out. You have to have reached floor 20 of all 5 dungeons - Jupiter Enterprises, Sewers, Caves, Utility Tunnels and Laboratories. And enjoy the labs, it is quite unforgiving.
There is one catch to this one: You have to do all of this in one playthrough. Doing 20 floors in one game and 80 levels in 'New Game +' will not result in this achievement being unlocked.
Items & Loot
Boom Goes The Dynamite
Use a Grenade. Fully explained in the tutorial. Most pointless achievement in here...

Now It's Better
Equip an attachement. At the workbench in a safehouse, after selecting a weapon, click on one of the squares and a list of available attachements will be displayed. Pick any one and achievement done. Not all attachments fit all weapons so you may need to select a different weapon and fiddle around a little.

This Is My Gun
Pretty simple. At a workbench. Hit the rename button.

Tailor
At the workbench, select the gear you want to max out and hit the upgrade button until the bar is fully white. I recommend saving this for a bit. I have found it is really pointless upgrading weaker equipment. Wait until you get a rare item worth keeping or some heavy armour like the Spear RBA vest which you will keep for a while.

What Are The Odds
Find a rare item. Items come in 5 levels of quality, each with a coresponding name tag colour. By rare, this means a purple, tier 5 item. You are guaranteed at least 1 on the 20th floor of any dungeon, and are likely to get at least one other on the way there. Most likely in Jupiter Enterprises, you will find a 'Skorpion' SMG before you hit floor 20, but at floor 20 there you will get the 'Breach and Clear M4' and the 'Patriot SOPMOD'

Quartermaster
Master Of Upgrading
Combine these 2 achievements at once. Fully upgrade a weapon and fully upgrade a rare weapon. Would be easier to just upgrade one rare weapon and do both in one go. I did not do this and just wasted a few thousand scrap on lesser weaponry. You are only really going to keep the rare weapons, so those are the ones worth investing in. As above, this is likely to be the Skorpion SMG, which costs 5500 scrap to bring to its full potential. You upgrade at the workbench, just the same as Tailor, but with weapons.

Kitted Out
Equip each of your soldiers with a rare primary weapon. As above, these are the ones with the purple name tags. Even if you are not going to keep them, just equip them briefly before scrapping them to unlock this achievement.

Rare Loot
Equip a rare uniform, vest and headgear. Not difficult to achieve, but requires some patience. Just dont scrap any rare gear until you have had it all equipped. I'm not sure if you have to have it all equipped at once, but I did that, to cover all the bases
You are guaranteed to find a rare headgear on floor 20 of the caves dungeon - Sheriff's Hat. Not a good piece of equipment, but good enough for the achievement. The uniform I used was a PCU Jacket found in the Sewers. I think it was a random drop on floor B11. I got the armour in the Abandoned Laboratory where I picked up a rare vest - Plate Carrier. I actually got 2 here in one run.

Looter
You do not have to try to get this achievement. It will come to you. Just keep playing and looting. Keep an eye on the minimap for the yellow chest icons.

Scrapper
You will want to scrap all the gear you pick up but do not want to use, so this will also happen without trying for it. Quality does not matter, so even the common white weapons count.

All That Glitters Is Gold
You need to have accumulated 50'000 scrap. I tended to spend it as fast as I got it, so I do not think you needed to have it in you inventory in one go, just have collected that amount over time. You should unlock this achievement pretty fast.

'Defeat' Achievements
Cannon Fodder
Kill 100 Roamers. This will happen very quickly and without you trying.

Restraining Order
Kill 100 Stalkers. Same as above. Very little difference between the two achievements.

A Quick Death
Kill 100 Rushers. Not a difficult achievement. These appear in more numbers later in the game, so may take a little while to unlock. Pin or Suppresion are effective against these.

Projectile Vomit
Kill 100 Spitters. This just takes time. They can be found in greater number in the lower levels of the Sewers and Utility Tunnels. I found them in lesser numbers in the Caves. They are not a threat in themselves, but will probably cause some damage to your team, lessening their chance against other enemies. The best advice I can offer against these is to make sure that your team medic has the Healing Aura skill to mitigate this light damage.

Hunt Or Be Hunted
Kill 100 Humans. Just use a Sniper Rifle and these guys are not very tough. Combined with the Fireteam Leader skill where you cannot miss and even those in cover do not pose a problem. Explosives are the usual tactic though.

Shoot The Medic
Kill 100 Breeders. Alone these guys are big and tough, but pretty ineffective. Using Pin or Suppression to keep them away from groups of downed enemies makes your life easy. They are only found in the Parks area and later, so it may take a while to build up the required 100 kills. Probably one of the last achievements you will unlock.

Grim Reaper
Kill 1000 enemies. Just line them up and knock them down. Repeat until unlock....
Weapon Mastery
AR Master
Shotgun Master
LMG Master
SMG Master
Rifle Master
Sidearm Master

You only need to defeat 50 enemies with each of these weapons. It does not need to be the character you directly control, so feel free to equip one of the 'followers' in you team with a weapon from the needed category until the achievement is unlocked and then move on to the next. Relatively easy to get these six achievements done fast.
Multiplayer
Two Heads
Best Buddies
Fight Together

I would love to be able to comment on these, but I suffered game breaking bugs and had to abandon attempting these. Good luck with these, but I do not imagine them to be too difficult to unlock.
Miscellaneous
I Love It When A Plan Comes Together
Execute 100 orders in command mode. Just as long as you dont try and play the game in real time, you will probably get this in your first sitting.

Halfway There
You will unlock this achievement as soon as one of your team reaches level 15.

Max Power
You need to reach level 30 with one character for this achievement. If you are grinding to unlock this, the 'Experienced' skill from the 'Intelligence' skill tree will certainly be useful on one of your team. With or without this skill, it still takes a fair bit of effort to get this achievement and is one of the reasons you will need to play it through more than once. Remember you get bonus xp for clearing an AO, so that helps when grinding levels.

Bite The Hand That Feeds
Yet another reason to get to floor 20 of the Jupiter Building. Located in the last room of this floor, after the fairly hefty brawl, you will see 2 loot chests containing rare weapons and right in the middle is a giant sized framed painting. Use manual fire to place some rounds in this and it will give off some mini explosions, as it drops off the wall. This unlocks the achievement.

Call Me Sccop Life
A very simple achievement to unlock as the main quest line takes you very close to it. All you need to do is speak to a person called 'The Philosopher'. The trick behind this one is that this person is hidden. You will be asked in the main questline, after a quick visit to the Utility Tunnels, to go to the fire pit. This is right next to the chapel. On the other side of the building, hidden amongst the trees is where this character is hidden. It is only accessible one way, despite the map showing two. You only have to listen to one line of philisophical speech to unlock this, but if you want, more is available.

Conclusion
Just focus on making sure you get through to floor 20 of each of the dungeons and leveling up your team. Everything else sort of falls into place as you go. The only exceptions to this are the multiplayer and miscellaneous achievements that will require you to pay attention and go to a little effort.

Just a side note - MP achievements cannot be obtained as the developers removed the whole MP feature from the game.
