Preface
Since I haven't seen a guide for getting 100% on this game yet, I've decided to collect some thoughts on how I got the achievements and write them all down here. There are also two videos that helped me a lot, so I'm leaving them here as well. I do not own the videos in this guide. All in all, this is a brief collection of what-to's and how-to's.
Storyline achievements
These are the achievements you'll naturally obtain while playing the game. You are guaranteed to get these once you complete the whole storyline. If you don't want to get spoiled the story, scroll down to the next section without reading this. You have been warned.

Free Range - Find Sanctuary in the Holy Caves.
Obtained upon entering Monsaic Lines.

Spirits' Unrest - Acquire the first hand scar.
Obtained upon getting through either Paramonia or Scarabia.

Shrykull Awakened - Acquire the power of Shrykull.
Obtained upon getting through the land you picked second.

Employee of the Year - Break back into RuptureFarms.
Obtained upon reentering ZULAG 1.

The Final Hour - Shut down the RuptureFarms generator.
Obtained upon pulling the generator lever at the very end of ZULAG 4.

Liberator of the Mudokon - Become a Mudokon saviour.
Good ending. Obtained upon completing the game with at least 150 Mudokons saved.

Mudokon Pop - Become a Mudokon salami.
Bad ending. Obtained upon completing the game with below 150 Mudokons saved.
Playthrough achievements
These are the achievements you should be getting while playing the game, but you might have missed out on them.

Speed of the Mudomo - Escape 5 or more pursuing Paramites.
This can be obtained in one of Paramonia Temple's challenge rooms (right side, up-left one). When you pull the lever to lit the torch, a lot of Paramites will rappel down and to progress you will need to escape them.

Speed of the Mudanchee - Run from a Scrab for 25 meters and survive.
There is a secret area with a few Mudokons at the beginning of Stockyards in which you'll have to run from a Scrab bigger distances than that.

Close Shave - Escape a Scrab by the skin of your loins.
Can be easily done in the very same room as above. Rappel down the platform you're starting at, but don't touch the ground. Wait for the Scrab to come at you and try to time down the last moment possible to climb up.

Hit the Deck - As a possessed Slig, fire over the head of a cowering Mudokon.
While possessing a Slig, you can tell your fellow Mudokons to duck, so they don't die while you shoot in their direction. This command has the same button as "Wait" on Abe.

Mine Games - Deactivate 3 toggle mines in a row with no mistakes.
In one of Paramonia Temple's challenge rooms (right side, up-right one) there are three toggle mines at the right side of the map (one at the ground, two on a platform). Arm the landmine below and disarm it, then proceed to disarming the two landmines above.

Close Encounters of the Slig Kind - Kill a Slig with any kind of mine.
You'll surely get this in some point of the game, there are numerous situations where you can kill a Slig with a mine.

Cooked To Perfection - Destroy a Chant Suppressor with a cooked grenade.
Can be easily obtained either at the end of the game or earlier secret areas.

Odds and Rockers - Bounce a rock off a wall and on to a mine.
There is a secret area portal in Stockyards that's blocked by a couple of mines at the wall. You can bounce your rock off the wall to get that.

Under New Management - Drop 2 Sligs down a single trapdoor at the same time.
In the middle of the first chapter, there are two Sligs near the same trapdoor. You can kill them both when timed right (bottlecaps can help in this).

Mudokonga - Rescue 9 Mudokons at the same time.
Some secret areas have more than nine Mudokons in it, just make sure you send them home together.

Don't Get Mad: Get Elum - Ride Elum for 200 meters without stopping.
Should be obtained when getting through Paramonia or Scrabania, there's a few parts where you have to run long distances with Elum.

Nine-Tenths of the Law - Kill 9 Sligs with a single possessed Slig.
At the beginning of ZULAG 4 after going through the first door there is a lot of Sligs wandering around. Possess one and the achievement should pop when you kill them one by one carefully.

A Long, Hard Slog - Kill 30 Slogs in 30 seconds.
At the end of ZULAG 4 there is a lever that you have to pull in order to progress, and to get to it you'll have to kill around thirty Slogs leaving their doghouses.

Oops… - Fail to observe your surroundings.
This can be obtained by leading a Mudokon to his death. Getting him killed by a Slig doesn't work.

Rockin' Mudokon - Kill a bat with a rock.
It's very likely you missed that achievement if you didn't know about it. Chapters that contain killable bats are Stockyards and Free-Fire Zone. There's plenty of rocks to work with, just keep trying.

Why Do Birds Suddenly Appear…? - Have Abe die in 15 different ways.
You should get that in your first playthrough. There is a lot of ways to die in this game.

RuptureFarms: Now Hiring! - Kill Sligs in 15 different ways.
There is a high chance that you missed this one. There is a total of 18 ways to kill a slig and they are shown nicely in the video below, props to PS4Trophies for that.

Completion achievements
These are the achievements you'll get while rescuing all 299 Mudokons. There is a great videoguide by PlasmaSKX, so if you're missing on something, you can help yourself with it. Alternatively if you want to track your progress and check if you're not missing anything, you can see the percentage of rescued Mudokons in Chapter Select.

Disciple of Odd - Rescue a quarter of all the Mudokons.
75 Mudokons escaped.

Shepherd of Odd - Rescue half of all the Mudokons.
150 Mudokons escaped.

Prophet of Odd - Rescue every Mudokon while finishing New 'n' Tasty in Easy mode.
IMPORTANT: This achievement can also be obtained by rescuing every Mudokon on Normal and Hard mode.

Savior of Odd - Rescue every Mudokon while finishing New 'n' Tasty in Normal mode.
IMPORTANT: This achievement can also be obtained by rescuing every Mudokon Hard mode.

Odd Messiah - Rescue every Mudokon while finishing New 'n' Tasty in Hard mode.
There's no way around, gotta do what's in the description.

No Place Like Home - Find and use all the secret entrances to RuptureFarms.
There's seven of them in Stockyards, one in Paramonia Temple and three in Scrabania Temple. Look at PlasmaSKX's guide at the top of this section for reference.
Challenging achievements
These are the achievements that will require some effort on their own to get. Act of Odd and New 'n' Hasty require replaying the game, while the others can be achieved by just replaying a specific chapter.

Act of Odd - Kill 99 Mudokon slaves.
Bringing the casualties counter to 99 does the work. At the beginning of the game you can leave 70 Mudokons if you don't save anyone from RuptureFarms, then killing another 29 can be done in Stockyards' secret areas, or in the rest of the game if you're aiming for the bad ending. At first I thought you have to kill the Mudokons on your own (like in the "Oops..." achievement), but it turned out that it's the casualties counter that matters.

Still Life - Escape Stockyards and Free Fire Zone without triggering the alarm or dying.
The description is misleading, because you need to do that in just one of these chapters. Stockyards chapter is somewhat easier, just quicksave frequently and be careful, you don't have to hurry.

Valve Time - Turn off the gas with more than 30 seconds to spare.
In The Boardroom chapter you must have more than 30 seconds at the time you pull the level that summons two Sligs that capture and drag you away in the ending cutscene.

Escape Fartist - Escape from RuptureFarms without Abe being spotted by any Slig.
You are allowed to chant, possess Sligs (as long as you don't get spotted) and even die, as long as it wasn't a death from a Slig.

New 'n' Hasty - Rescue every Mudokon with an overall Best Time of 3:00:00 or less.
I played on two parallel save states: the one on which I explored, and the one where I rushed when I knew everything. I highly suggest doing the same unless you want to challenge yourself and do everything in one sitting. Can be done on any difficulty.
Postface
Well, that's it. In case you are here to find all the Mudokons, I encourage you to find them all on your own with the help of Chapter Select menu, where you can track your progress and focus on exploring. However keep in mind that if you pick a chapter, your progress will be deleted.

Good luck!

I unlocked "No Place Like Home" with chapter select (within the same save profile), so it seems like not necessary to do it in one playthrough

For NO PLACE LIKE HOME: Do you need to have them discovered in one play through or can you go back and open the portals later?

Act of Odd - Kill 99 Mudokon slaves.
"it turned out that it's the casualties counter that matters."
If it was so then it would be enough JUST make it to Monsaik Lines without saving anybody. And I did it and also Paramonia. No achivement. You either have to see them die or may be at least you must visit secret locations and then leave them without saving anybody. Don't know exactly yet.

Still Life - Escape Stockyards and Free Fire Zone without triggering the alarm or dying.
"The description is misleading, because you need to do that in just one of these chapters."
No, it's not missleading. I just did Stockyards, no achivement. In another discussion people say you have to not just avoid everything but also do it without saving. Which is totally possible on Stockyards and almost not on Free Fire Zone.

Still Life - Escape Stockyards and Free Fire Zone without triggering the alarm or dying.

"..just quicksave frequently.."

Atleast on normal difficulty, I did not get this achievement if I died or triggered an alarm and loaded a quicksave. I tried it again restarting the whole chapter after death/alarm and got the achievement upon finishing Stockyards chapter.
