The issue can be that...
-Game Files Missing
-Game Errors. (Check Console For LUA errors)
-Addon Not Working Correctly.
Fixes~
*Re-install The game.
*Delete all addons that you have downloaded from workshop.
*Go to Game Directory The Folder where all the Game Files Are Located And Delete everything Even the "Garry's Mod" Folder in "common" folder.
*Use Google.

I recomend removing all addons you got on garrys mod and then re install the game so its fresh new.
