Star Decay is a 1% penalty to the total stars per chapter after 60. For example, if a game lasts 65 chapters, and the total before Star Decay would've been 1000, then Star Decay will take away 50 (5% of 1000), and you'll get 950. (I'm unsure if Star Decay would actually take away everything at chapter 160, as the longest game I've played went to chapter 98, which cost me 38% of the bonus.)
I believe Misfortune is related to landing on a large number of traps, although I'm not 100% sure. I've sometimes gotten Misfortune following trap-heavy games, so that's what makes me think this. And, I've had Misfortune bonuses of 100 and 120, so it probably varies depending on how many traps you trip, just like how Card Master and Trap Meister can vary.
Also, I don't think the boss is limited to just 4 extra summonings per game.
And yeah I realized there isn't a difficulty bonus (I could've sworn I saw it though...) and forgot to remove it.
1) There's no difficulty bonus. You get the same stars when you beat a campaign on any of the 3 levels. I've just tested it.
2) You forgot to mention the Extra Scenarios: "Extra Scenarios count as Episode 7 [worth 350 stars], while Tomomo Hell counts as Episode 10 and grants you 500 stars"
