To get all achievements in Lords of the Fallen you will have to finish this game at least 3 times with the same character. That's because each ending grant's you a soul-bound item needed for achievements.

Playthrough 1 - Adyr Ending, most Side-Quests and Collectibles

In first playthrough you want to concentrate on Adyr Ending meaning you can't interact with any Beacons. Beacons are those red glowing pyres. DO NOT INTERACT WITH THEM ON THIS PLAYTHROUGH!
Follow my All Collectibles videos to get all possible collectibles and side quests.


Playthrough 2 - Radiant Ending and Kukajin Quest

In this playthrough you MUST cleanse all Beacons.
Also in this playthrough you should do ukajin quest that was canceled in Playthrough 1.


Playthrough 3 - Umbral Ending, Paladin Isaac Quest, Iron Wayfarer Quest and Defeat All Bosses

In this ending you must kill all NPCs and you can't interact with single Beacon.
