How to unlock the "Items Descriptions" achievement

Today, as of 10th of September 2025, a new achievement popped up, ruining many 100% completions.

This brief guide will tell you how to unlock the new achievement, fair and easy.

What was added in the update?

The pause menu got shrinked and moved to the bottom of the screen.
It also came with a new section called "My stuff".

Once you open it, you can read your items' description.

What to do to get the achievement?

The new "My Stuff" section can only be unlocked when defeating Mom (the foot) for the first time.

That means that if you want to get the achievement you just need to do a run and beat Mom once, at least that's how I got it.

Sadly, that won't be the case for everyone, since there's more to it...

This is official news from Edmund McMillen that states that you also have to collect every item at least once to unlock every description and, probably, also the achievement.

Indeed, most of the people that managed to get this achievement, already had Dead God.

Also, this achievement is probably only obtainable in Repentance+.

All of this is only hypothetical, at least for now.
There is still mystery around the achievement and how it actually works but this is all I have for now.
This is the most trusted way you can choose to get the achievement, but it's not 100% guaranteed.

All you've just said I already knew.
I specified that you might have to collect all items because people would say they didn't get the achievement by only beating mom's foot.
I know you have to do it regardless of your progress, but sadly there's still some mistery behind what ELSE is asked to get the achievement.

also- it doesn't count previous times you've beat mom's foot for the item description achievement. you have to do it once again regardless of how far you are in game

yea Drip. from what I've noticed in-game you only need to beat mom('s foot) to get the item description to unlock and then it shows on a item by item basis. when you come across an item you havent collected once yet the item description shows a ? mark and tells you to pick it up to unlock the description for that individual item. and this was all tested on a save file that is only up to unlocking the negative (I don't even have the polaroid or alt paths or many characters unlocked on this save)

KyloRennifer, I see you have the achievement without having Dead God, this changes everything.
This is still very unclear to me, but I'll update the guide.

you don't need to have touched every item in the game to start using item descriptions. you unlock the description for each individual item you pick up (after beating mom, yes even if youve beat her many times you gotta do it one more to pop the achievement)
