Collect all the clues. See the map as a guide:

Only triggered for me on the second run (bug?). You can spam F to speed up conversation.

I don't know if you can die, but I assume you can as long as you complete the levels.

Beat the game with the true ending.
Will unlock if you don't kill any victim (Collect all clues, see map above).
'-> You were not enough
Beat the game with the bad ending.
Will unlock for certainty if you kill all victims. Potentially will unlock if you kill only one (untested).
I've skipped every dream by running towards the enemy and unlocking the exit on respawn, and the achievement still worked. I did not skip meal delivery.
