Usually, after your squad travels to unknown places, some of the rangers may get sick, but if you still can't start an epidemic, there is another option with the plague doctor: Take him into your clan, let him work, and then find out that half of your people are dead, and the other half are infected.

I received this achievement after one of the clan leaders left me.
At that very moment, I had an epidemic, the inhabitants were dying without medicine, and in all the clans, the excitement rose to the maximum due to grief, so perhaps this somehow affected the achievement of the achievement. There is also an option - to settle residents in different areas. But I got this achievement immediately after one of the leaders left me, I don't know if it's a coincidence or not, but I'm inclined to the option that the achievement is associated with the departure of the leader and a lot of excitement in other clans, and not with something else, but I may be wrong.

The only difficulty will be the power plant, because without completing all the tasks related to repairing it, you will not be able to pump "Electricity" and the rest of the branches inside with it.

What you need to get this achievement: be sure to build a police station, I went bankrupt on this twice. You need to keep an eye on the crime rate, because if it's even a little high, then you start stealing and fighting. The more people, the more police stations are needed. In addition, you should have a "Clinic" built, preferably two of them, because by 200-400 days people begin to get sick very often, and if you do not monitor this, they will begin to die out due to the epidemic, do not forget to stock up on medicines as well. With water, usually, there are not much problems, just install the "Water Purification Plant", personally I had a "Water Purifier" x4 and a "Water Distiller" x5, almost always there was 500+ water, even without any laws. Be sure to build a "Shellfish Farm", because very often there is an invasion of giant fish, so the "Fisherman's Wharf" will not work for a while, and therefore your people will start starving. I didn't really develop mushrooms and the bakery, since I had enough food for everyone from the "Shellfish Farm" and "Fisherman's Wharf", just don't forget to build a "Field Kitchen".

Keep an eye on your actions because some of them can lead to the death of your people, for example, a random event when trackers search a house, very often they can tell you that they have a beehive, a skeleton, a scary place, etc., because of some options your inhabitants can die. For example, if you hit a hive and go further, you will get a lot of food, but at the same time three of your people will die Maybe randomly, but I had it. Or, for example, the random event "Plague Doctor", if you take him to the clan, then he can take several of your people for experiments, after which a decent number of people will die and become infected.
When you send rangers on a journey, do not forget about them, if you forget, they will simply die of hunger, or if you are too greedy, and when your people have 0 rations left, you will continue to explore the buildings, they simply will not be able to swim home.

You need to hold out until the final storyline, that is, provide 150 inhabitants with electricity, then you will be given an achievement.

You must have 150 inhabitants, without this achievement you will not be able to get. My advice to you - until the end of the game, try not to collect people who are on other islands. Be sure to keep an eye on the number of people, because usually the most "fun" time in your clan begins on days 200-400. In my first game, I immediately took all the clans to me and gathered all the people on other islands, as a result, by 200+ days they began to get sick all the time, they were hungry, rebelled and stole, and then they also died and somehow by chance it turned out that at the end of the game I did not have enough people.
