Story Related Achievements

You will unlock all these achievements by just playing the Story Mode.

First False Step

You took the first wrong step. It will be remembered.
You have to make a mistake and take the pills or open the door when you aren't supposed to.

No Escape

You tried. But this place doesn’t let go.
You have to survive the first 9 days straight until the escape cinematic pops.

Bori's Truth

Some truths aren’t hidden. They’re just waiting.
You have to find the 3 pieces of Bori's doll and then end the 9 days straight until the cinematic pops.

He Was There

There was another story. You just didn’t want to see it.
You have to find the 3 pieces of Takgu's doll and then end the 9 days straight until the cinematic pops.

Her Hidden Truth

You pieced together the fragments she left behind.
You have to find the 3 pieces of Ms.Gomu's doll and then end the 9 days straight until the cinematic pops (it might happen before you finish the 9 days).

End of the Endless

You found the truth, not to blame yourself — but to understand.
Once you collect all the pieces of the dolls and end the 9 days straight you'll see a cinematic.

Hidden Achievements

There are 4 hidden achievements that you can ONLY discover playing Story Mode.
A random day you'll wake up with a broadcast playing on your TV, once it's over 1 of the 4 hidden hallucinations will unlock. You can repeat this day as many times as you need in order to unlock all of them.

Dharma Is Shy

You stared too long. He noticed. Then he vanished.
Once the broadcast is over, watch the Dharma painting on the wall for a while until you can see his eyes pop.

I Saw Myself

It looked like you. But it wasn’t blinking.
Once the broadcast is over, go to the bathroom and clean the mirror, once it's all clean open and close the toilet for a few times and watch your reflection appear in the mirror. Repeat until it happens (it might take a while, if it doesn't pop try clicking the mirror).

Ritual of Flicker

You performed the ritual. Or maybe it performed you.
Once the broadcast is over, turn on and off the lights until they turn purple.

Broadcast Error

This scene was never broadcast.
Once the broadcast is over, turn on and off the TV until you see an image pop.

Endless Mode Achievements

This is Endless Mode and you can play without Story Related events which makes the gameplay more frenetic. You'll only have to survive withouth diying.

25 Days: Unnatural

Survived 25 days in Endless Mode. Respect to you, who never gave up.
As the achievements says, you only have to survive 25 days on Endless Mode.

50 Days: Inhuman

Survived 50 days in Endless Mode. Your perseverance doesn’t fade easily..
As the achievements says, you only have to survive 50 days on Endless Mode.

100 Days: Redacted

Survived 100 days in Endless Mode. That's not perseverance—that's a legend.
As the achievements says, you only have to survive 100 days on Endless Mode.

Hallucinations Achievements

It Wasn't Real (x10)

You died 10 times believing in something that wasn’t real.
As the achievement says you only have to die 10 times, it counts Story Mode and Endless Mode.

Hallucination Master

You found every hallucination. You looked through the cracks in reality—and never looked away. Now tell us... What was real?
As the achievement says you only have to discover all the hallucinations in the game. There are a total of 62 hallucinations + 4 hidden hallucinations (these doesn't count for the achievement).

It Wasn't Real (x10) won't work for me

The hidden hallucinations will also work in Endless Loop Mode, but you may be in for a long wait for a broadcast! Ritual of Flicker will also sometimes work if you flip on and off the light after briefly seeing the babydoll or scissors & forceps inside the light. i think the key is having a short-term vision in the vicinity of the light. Same goes for the Dharma. i've personally never triggered him with anything but the babydoll inside the light. As for I Saw Myself, so far i've only been able to trigger that one after first seeing the stethoscope appear temporarily in the foyer. Again, a short-term vision somewhat near the trigger area may be the key.

Side note, maybe some of these simply behave differently in Endless Loop. Either way, you'll get the achievements.

I can't get the "First False Step" to work. I'm making mistakes even on purpose so that it pops but nothing. I just need this and the 50,100 days for the 100%. Is it possible that it bugged because I had the demo?

Keep in mind that the 4 hidden anomalies can repeat - so there is no guarantee that just going through the game (which will include 4 broadcasts) will unlock them all - you may need to keep trying.

For Dharma is Shy, I got it without a broadcast. A couple of piano notes played when I turned off the lamp, and after a little while his eyes changed.
