Bowl of Revelations is located relatively far down in Pilgrem’s Perch and can be reached quickly from Vestige Point Belltower if we follow the path from the elevator to the left. Once we have found the quest item, we have to return to Molhu in Skyrest Bridge and give him the item so that he can expand his dealer inventory.

  • Without Purpose
    Thehk-Ihir leaves Mournstead.

For this quest we have to talk to Thehk-Ihir at all possible points in the Story. If we ignore him the quest will fail. With two exceptions, Thehk-Ihir usually stays close to Vestige shrines.

  • Antanas' Legacy

To do this, we have to bring a Book back. This can be started at any point in the game and cannot be missed as long as we do not go into New Game+.

  • Travels Resumed
    Sparky is liberated.

Give the last rune tablet to Sparky to free him from the blacksmith. As a reward we can use the services of the blacksmiths at every Vestige.

  • Hero Worship
Drustan’s faith in his brother remains steadfast.

For this quest we have to talk to him at all possible points and, most importantly, light the campfire in Fief of the Chill Curse so that the quest doesn’t fail. The quest is complete once we see his last stigma.

This quest only works while going for the Adyr Ending.

  • The Last Step
The Iron Wayfarer’s journey finally ends.
For this achievement we have to talk to him at every point we meet him. This quest works only if you’re going for the Umbral Ending!

  • A Trace of Venom
Kukajin concludes her business in Mournstead.

Kukajin’s Quest is relatively simple. We free her with a Radiant Healing spell so that we can summon her in boss fights. We have to make sure that we pay her after every boss fight, otherwise she will kill us and the quest will fail.

  • Lingering Moments
    View all Umbral stigmas.

There are 60 Umbral Stigmas in the game that we have to see. Umbral Stigmas are tragic events from the past that happened in Axiom. To see Umbral Stigmas we have to look for blue moths in Axiom. You can set out to avenge a player by Soul Flaying a red lamp. These appear on the ground nearish to where the player died. Once you soulflay the lamp, a red line of butterflies will guide you to the enemy.
It is easiest to backtrack from a vestige towards the previous one, because the enemy must still be alive in your world as well.

  • Umbral Offerings
Donate a total of 10 Plucked Eyeballs.

To do this, we have to farm the red lamps on the ground. If we successfully avenged a player, we get 3 Eyeballs. Once we have done this 4 times, we can go to Skyrest Bridge. We can donate these in Skyrest Bridge while in Umbral at the Shrine of the Putrid Mother.

  • Radiant Offerings
Donated a total of 10 Pilfered Coins.

To do this, we are either invited by friends or select Multiplayer on Vestige and the item Accompany Lampbearer. After defeating a boss together, we get 15 of the coins. We can donate these in Skyrest Bridge at the Shrine of Orius.

  • Infernal Offerings
Donate a total of 10 Severed Hands.

To do this, we open the multiplayer mode at any Vestige and select the Slaughter Lampbearer tab. After a short time we become invaders and have to kill another player in PvP. This immedietely answered my question whether you need to do multiple playthroughs on a single character or could do the achievements across multiple characters, so thank you.
