The ultimate persona are powerful persona only available near the end of the game, they can be fused by adding certain Totems to fusions. The totems are gained toward the end of the game, though the exact method on acquiring the totems depends on the route and will be listed below:

In the SEBEC route, the Totems needed to fuse the party's Ultimate Personas are obtained in a special way. The protagonist is given choices throughout the game that test his strength of character. You will receive a certain number of Totems depending on the number of correct answers: five if they're all correct, only two (for himself and Maki) if there's any incorrect answer, and zero if they're all wrong. The answers are as follows:

In the Snow Queen route, the protagonist has to trade Ambrosia gained from defeating the Tower bosses with Raiho during the final dungeon to obtain the totems required to fuse the persona. If the harder towers are completed before the easier ones, more Ambrosia will be given.
For all 5 ambrosia you must complete them in either order:

Thanatos Tower > Hypnos Tower > Nemesis Tower
Thanatos Tower > Nemesis Tower > Hypnos Tower

It is recommended that you set up before entering to the shrine near the start of the game. A useful guide to this can be found here: http://persona1.wikidot.com/sqqprep Essentially you want to level up to around 20, purchase move tablets from the casino, use them to help create more powerful persona and save around 500,000 Yen before entering the shrine.

Once inside Thanatos Tower use Contact to avoid big battles.

Note: To acquire the ultimate personas you must be on track for the good ending which requires you to obtain at least 8 mirror shards. The locations of the 12 Mirror Shards are listed below:

Lastly, late into the game on 4F of the Ice Castle, There is a specific pitfall (1st picture below) that'll land you on 3F near a room. Where inside you can exchange all the Ambrosia you have obtained for Totems that you can use to fuse the ultimate Personas for your party. First speak to Raiho (below) for the MC's Totem then speak to your party members for their own specific Totems.

The ultimate Personas are fused by adding certain Totems to fusions that result in a Persona whose Arcana is the same as the ultimate Persona's Arcana. They can only be equipped by the character they belong to.

Obtaining Vishnu the most powerful persona of the protagonist requires fusing the Spell Cards of Gandharva and Yaksini with the Avatar Totem. The Avatar totem is received automatically after obtaining all 142 Spell Cards from each demon in the game.
