This was by far the most efficient guide I've ever followed, thanks sir!

The Eldritch Madness achievement is for going over the otherworldy influence limit, so if you pick a class with higher wisdom, it won't trigger at the second altar.

After doing some tests I can confirm that, in order to find the Moonlight Blade, to make the crystal bridge appear and be able to cross it, you need to fill the bar of otherworldly influence to the maximum towards Abyssal, the bridge will appear when the bar is almost full, but it will become traversable only when the bar is at the maximum. The bar can be filled easily by selecting the third option in the first 2 altars (cleanse the altar). This will also unlock the Eldritch Madness achievement, since you will effectively break the limit of otherworldly influence, which by the way is filled not only with the altars, but also by repeatedly casting spells of one type and using a weapon aligned to one of the two otherworldly influences.

it has nothing to do with the altars and everything to do with alignment. You need very high Abyssal alignment.

@MemeMachine try what NectoJ did, create a new save and clear the first two only

I can confirm the bridge isn't spawning for me as well, I did clear all of shrines, so maybe it has something to do with the alignment?

@Skoord I tried that too. Quit and then started the game anew, loaded the previous save file where I completed 6 altars and then went back to look, funnily enough the bridge still didnt appear
