The scoring for treatments confuses me. I sometimes go with the highest value shown for the diagnosis. But the game then tells me I picked the wrong diagnosis. Then why did it give the one I picked the highest score then?

Only you can't "just" remove clothing.

You have to trigger it with another action like a bandage, which is pretty dumb.

like you do in real life , remove clothes from the patient so you can clearly see his body .
