TL:DR This mod is basically Hard 12 (NON-mutation)

This difficulty is balanced for 8 Player Co-op

What changes from Expert to 8 Player Expert?
• Common Zombie Population: 30 -> 40
• Common Front Hit: 20 -> 40
• Common Back Hit: 10 -> 20
• Special Infected Timer: 45 -> 30
• Fire Damage: 1 -> 1.5 (damage to players caused by gascan flames)
• Tank Health: 8000 -> 18,000
• Witch Health: 1000 -> 2000
• Special Infected Limit: 2 -> 10
• Special Infected Standby: 2
• Special Duplicate Limit: 1 -> 3 (How many of the same SI are allowed to spawn)

Why? Because 8 Players (especially good ones) exponentially overwhelm zombies by sheer firepower alone so, though extreme, these settings are adjusted for a Expert 8 player challenge

While not required, its HIGHLY recommended to have the ''Double Tanks in Finales''

You can still play this regular 4 players but for a more fair challenge full 7-8 player lobby is recomended for the best experience posible.

restart l4d three times with no other mods and I still haven't got it to work again

Yeah, sometimes it has five difficulty, sometimes it only replace the regular expert name for some reason.

@Nightpigeon reset l4d2 if it doesnt work. if you mean 8 actual players thats apart (its an external addon) you can install it yourself with this tut

the usual expert difficulty is missing, only the 8 player expert being show unlike in the thumbnail.

something is broken and I don't know what. The additional bots doesn't show up when I play first map of Hard Rain, multiple Jocky spotted though.
