Secret characters are "secret" in that their unlock conditions are only present in the secret menu, and said menu only gives somewhat ambiguous hints. The unlock methods include killing certain (hidden) enemies and finding Easter eggs in reference to other video game franchises.

Toastie: You will only be able to unlock this character after Exdash are unlocked, killing any of the following: The Stalker, The Drowner, or The Trickster |tramezzini.
Smith IV: You will only be able to unlock this character after Exdash and Toastie are unlocked
Type "spam" in the main menu - this will start a hidden 30-second timer. Go to the character selection and enter "spam", go to the stage selection and enter "spam", and enter a run and enter "humbug". When you leave the run, Smith IV will be playable.

Obtain the Yellow Sign.
Enter Mad Forest and remove the Skull O'Maniac and Pummarola stage items. This can be accomplished by picking them up (even if they are discarded after) or by obtaining them through leveling up and then maxing them.
When these conditions are met, Pies will start to appear and lead you to a specific part of the map (left on the grid of the Hollow Heart and above the Metaglio Left), where a shadowy enemy will appear; it will slowly move toward you even if you do not go there. Defeating it plays a jingle that signals you have unlocked the character.

Avatar Infernas:
After clearing Eudaimonia Machine by defeating the The Directer and obtaining Greatest Jubilee, enter Inlaid Library with inverse mode toggled on. Travel very far right past the Gold Ring, until you see The Trickster in front of a piano. You must kill the The Trickster while he is at the piano: If you stray too far away from the piano, The Trickster will relocate, and your attempt won't succeed. Once you've killed The Trickster (at the piano), interact with the piano (just ram into it); Doing so will pause the game and open up an interactive pop-up menu with clickable piano keys. If you have Peachone and Ebony Wings (Vandalier and Cosmo Pavone's birds do not count), the birds will show you which keys to press. To avoid evolving Peachone and Ebony Wings into Vandalier you may wish to Banish one of the bird weapons after acquiring it to prevent it from leveling up to level 8.
You will find yourself in a dark area with large amount of Undead Heads and Undead Eyes and nine coffins. Travel around the area and find Avatar in one of the coffins (his coffin location is randomized). After opening the coffin containing him, all other enemies will disappear and he will start attacking you. Defeat him to unlock him in the roster. Once Avatar is defeated, the White Hand will appear and end your run.

How to unlock:
Obtain Yellow Sign.
Enter Dairy Plant and get the cheese, which is visible on the map as a Floor Chicken. It can also be attracted using Mad Groove (VIII).
Once collected, 7 werewolves will spawn. Defeating all of them plays a jingle that signals you have unlocked the character.

Leda:
To unlock Leda, follow these steps:
Start a run in Gallo Tower using a character with high movement speed (such as Red Death). Applying hyper mode to the stage is helpful for the added movement speed bonus.
Travel south until the screen is completely dark on the edges. You will know you are in the correct area when the music is replaced with ominous wind and the letters in the background change from runes to "IAMALIVEHERE".
Continue to travel south, zig-zagging to ensure that you see all areas of the floor. You will soon encounter Leda, who can be difficult to spot amongst everything else on screen with your limited vision. (She can also be hidden inside the wall.) Be careful not to pass her - she is too slow to chase you, and will not teleport to you if offscreen.
Defeat Leda to unlock them as a playable character. She has quite a bit of health, so it may take a while. Once you have defeated her, the screen will once again be illuminated, and a small jingle will play.

Cosmo Pavone:
To unlock Cosmo, the player must have already collected the Yellow Sign and unlocked Zi'Assunta Belpaese by opening her coffin. Then the player must enter Cappella Magna with a character with 0 eggs as part of the "pure heart" requirement. (Note: toggling eggs off does not count as having "0 eggs")
In Cappella Magna, the player must collect both Peachone and Ebony Wings (which must not be united into Vandalier). After obtaining the weapons, go north beyond the Crown and Tirajisú where a Nduja Fritta Tanto can be found near a balcony. Collecting the Nduja Fritta when all of the conditions above have been met spawns Cosmo on the balcony, and the gate blocking it is removed. Cosmo can then be approached, and a flash similar to Rosary appears.
Alternately, if The Ender has not yet been defeated, activating the Ender battle while standing near the balcony causes all the walls and gates to disappear, and you can walk over and collect Cosmo immediately after the battle.

Peppino:
On Il Molise, use Celestial Dusting to attack the plants, healing them instead. Upon restoring 100,000 health points to the plants, it plays a jingle that signals you have unlocked the character.
Using O'Sole will make this trivial, as it does not take damage from the plants.

Big Trouser:
In Moongolow, collect all 16 standard stage items and raise them all to max level. The character will be unlocked after the stage is left after fully upgrading all 16 items.
Because Tiragisú is lost once both of all the revives have been used, one attempting to unlock Trouser must leave the stage after all accessories have been leveled fully. Ending the run through death will result in not having a Tiragisú at the end.

Gains Boros:
Due North of spawn in The Bone Zone (beyond Silver Ring), there is a ring of flowers which heals the character by 8 every 0.5 seconds. By staying in the ring for 10 seconds, a jingle is played and the ring disappears. The healing from the ring will continue.

Gyorunton:
Gyorunton is unlocked after surviving 15 minutes in Boss Rash with only one weapon.
The easiest way to do this is to unlock the relic Mindbender, which allows you to limit how many weapons you can pick up. Alternatively, Game Killer (0) can be used to avoid leveling up entirely, so you can avoid taking more weapons. Running an invincible character like Ghost Lino[Emergency Meeting DLC] or Megalo Menya Moonspell[Legacy Of The Moonspell DLC] also lets you idle through the entire time.

As described by the developer: "Go to Astral Stair with any mode/character and step through doors 46 times. Doesn't have to be done in the same run, the counter keeps accumulating. Touch the coffin that appears after touching the 46th door."

Scorej-Oni:
Walk roughly 20 tilesets (map tiles) to the right on Tiny Bridge to encounter Scorej-Oni as an enemy. Defeat it unlocks it as a playable character. A sign that Scorej-Oni is nearby is the faint sound of thunder sporadically playing in the background, though this may be difficult to hear if there are many sound effects playing at once.

Gyoruntin:
Gyoruntin is unlocked by attempting to move left in the Carlo Cart stage. Even if this has no visible effect, Gyoruntin's coffin should eventually spawn, guarded by several Tri-Anchors. Defeating the Tri-Anchors then standing on the coffin will unlock Gyoruntin.

SPACE DETTE: unlocked by locating a special pillar with Pummarola outside of the normal playable area in Space 54, right from the beginning of the stage. Ferryman:
In the Ode to Castlevania stage, evolve the Sonic Dash into Rapidus Fio and walk onto the water near the castle's entrance.|hehhehheh.
Master Librarian:
Buy everything from the Master Librarian's shop at least once. In order to unlock and access his shop, the Wood Carving Score must be collected by defeating the Treant at the bottom left of the Ode to Castlevania stage next to the entrance to the castle's bottom path, all of which is accessible only after unlocking Vlad Tepes Dracula. Obtaining the Wood Carving Score then allows him to appear on any stage permanently.|rockingchair.
Hammer:

Wind:

Dmitrii Blinov: After unlocking Vlad Tepes Dracula, Menace appears from his summoning circle as a boss enemy in the Ode to Castlevania stage, located in the right side of the Sewer (underneath the first section of the front building) inside Dracula's Castle. Axe Armor: A certain number of Axe Armor enemies must be killed with the Stellar Blade, the evolution of the Discus. Keremet: After unlocking Vlad Tepes Dracula, Keremet will appear in the Ode to Castlevania stage as a boss enemy, at the top floor of the Laboratory (the top part of the front building). Defeating it as either "Jonathan & Charlotte" or "Charlotte & Jonathan" will unlock the character.|cauldronking.
Alamaric Sniper: A certain number of Alamaric Sniper enemies must be killed with the Stellar Blade, the evolution of the Discus. The top of the Cathedral (the top part of the far east building), as well as the bridge next to the entrance to the Clock Tower, are the best places to do this.|gothangel.
Blackmore: After unlocking the Scorpion Gate in the Ode to Castlevania stage, Blackmore will appear as a boss enemy from his summoning circle, located in the hidden section of the Library (inside the top part of the middle building). The room where his summoning circle is located in also contains an Arma Dio. Then, defeat him while playing as Barlowe.|bizarreshadeadventure.
Malphas: Malphas will appear as a boss enemy in the Ode to Castlevania stage at the 25:00 minute mark, and then defeat him while playing as Quincy Morris.|karasuman.
Death: In the Ode to Castlevania stage, enter the boss fight against Death (which is only accessible while playing as Richter Belmont) with Clock Tower equipped, which is the union of Endo Gears, Peri Pendulum, Myo Lift, and Epi Head unlocked after unlocking the Serpent Gate.|sewardzead.
Galamoth: After unlocking the Beast Gate in the Ode to Castlevania stage, Galamoth will appear as a boss enemy from his summoning circle located in the center of the Labyrinth. Evolve Dominus Anger, Dominus Hatred, and Dominus Agony into Power of Sire, and then defeat him with it. |sizematters.

• Location: Head southwest from the starting area, deep in the forest. Access via top or bottom entrance.

(2) - Behemoth

• Location: From the start, head northwest until you reach the top-left corner.
  
• Special Drop: Rock Riot weapon, essential for early progression.

(3) - Giant Bat

• Location: Cross the bridge to the east, follow the hallway until the rune appears.
  
• Reward: Unlocks the Stallion Gate, providing faster castle navigation.

(5) - Giant Medusa Head

• Location: After the Giant Bat, head east and climb the staircase to find the rune.
  
• Reward: Standard chest.

(6) - Slogra and Gaibon

• Location: After defeating the Giant Medusa Head, head west. These two bosses are activated simultaneously.
  
• Drops: Two separate runes, each with unique treasure.

(7) - The Creature

• Location: After the Giant Medusa Head, proceed north into the Marble Gallery and then west.
  
• Special Drop: Fulgur weapon, ideal for crowd control.

(9) - Doppelganger

• Location: Marble Gallery, in the northern region, where time pauses for a 2D duel.
  
• Reward: Chest.

(11) - Puppet Master

• Location: Northeast from the Scorpion Gate.
  
• Reward: Unlocks the Scorpion Gate, opening up new areas.

(13) - Paranoia

• Location: Southbound path from Puppet Master’s location.
  
• Reward: Unlocks Capra Gate.

(12) - Blackmore

• Location: Head west from the Paranoia rune through a bloodstained maze.
