Judging from the replies, this is a broken mod that doesnt work for any of half life 2's content. It also has been stolen with no credit, and is currently not taken down for some reason.

The addon works for me, even with all my addons/mods enabled, I don't really know then why a lot of users are getting such errors.
I just want to add that the model/textures don't cast shadows from the flashlight, that needs to be added, you could replace the model, or upload a new optional version with the improvement here
