Introduction
This guide is based in my walkthrough of the game. If you want to see a more detailed gameplay of the game or any of its parts, I recommend you watch my complete walkthrough:

All Keys Locations
D-Block - Code Locations
All 3 locations are on top of tables with metal plates. They are almost near each other and near the Corridors' door.

C-Block - Key Locations
  1. Room C-72, fourth floor. On top of a metal plate.
  2. Room C-77, fourth floor. On top of a metal plate.
  3. Supposedly on Room C-59 (jumpscare room), fourth floor. It is said that it is on the bloody floor.
  4. In one of the rooms of the same side as Room C-59, fourth floor. On top of a little white desk.

Infirmary - Key Locations
  1. On the "catwalk", laying on the ground at the right.
  2. On top of the white box.
  3. On the top of the desk, next to the old PC case.
  4. It is not confirmed that it has a fourth location.

Cafeteria - Key Locations
  1. Inside the fire hose reel.
  2. On the blood stain at the floor inside the kitchen.
  3. On top of the balcony near the door.
  4. It is not confirmed that it has a fourth location.
Good Ending
To reach the Good Ending, you just need to reach the door at the Main Entrance of the prison. The level design helps you to go forward this path. Probably you are going to get this ending in your first run.

Bad Ending
Solving the D-Block doors puzzle will open all the doors on the D-Block. After this, go near to the Spawn Room and look for a door at the left, on the walls. You will notice two doors: the Library door and a door with a "Security Checkpoint".

Go through the Security Checkpoint and you will find another door with a "X". Open this door to get the Bad Ending.

Dog Room - "Gamarra's Room"
Reaching this room will get you an achievement.

This room is located on D-Block, second floor. Near the stairs to third floor and near of the room with the D-Block puzzle. It has some Magic Cards on the floor and a picture of a dog.

This room is a Easter Egg of Lucas Gamarra, one of the game developers.

Poster Room - "Andy's Room"
Collecting the woman poster at the "Andy's Room" will get you an achievement.

This room is located on C-Block, second floor. It is the Room C-27.

Racket Room - "Unity's Room"
Reaching this room will get you an achievement.

This room is located on C-Block, third floor. It is the Room C-57.

This room is a Easter Egg of Lucas "Unity", one of the game developers.

Speedrun
You must complete the game in the Good Ending Route (I suppose) under 30 minutes. You can watch my gameplay if you are struggling with this achievement. Knowing all the Keys locations you almost guarantee you that you can finish the game under the limit. In this video, I was not trying this achievement and still got it. If you completed the game once, this will probably be easy.

D-Block Puzzle without missing any button

This is probably the most difficult achievement of the game.

THERE IS NO EASY WAY TO DO THIS.

You MUST get the correct order of the D-Block puzzle WITHOUT MISSING ONCE.

Completely LUCK.

There are a total of 24 combinations possible.
1/24 ~= 4,17%

You must focus on only getting this achievement in a run in order to complete it. Once you start a new run:
  1. Get the flashlight.
  2. Go to the left and get the key.
  3. Open the door.
  4. Search the first code in one of the three tables.
  5. TIP: if you just click on the colored note and skip it, the code will pop up on the HUD, in your left. In other words, you don't need to worry to memorize the code. Interact with the colored note and immediately close it.
  6. Enter the code on the panel.
  7. Try to solve the puzzle without missing.
  8. If failed, go the the Main Menu and start a new run.

Since the chances are equal to any of the 24 combinations possible, you can choose a fixed combination and pray for it to work every time. You will have a 4,17% chance of getting it right. If you choose to always try a fixed combination, your restart tries will be a little bit faster than normal.

I personally choosed the combination "1243". I got it on my 10th try.

I tried to check if there is any combination that is more common that the anothers, but it doesn't look that there is any. Just to avoid "easy" combinations like "1234" and "4321", I choosed "1243". But these two combinations are still possible, as I said.

Each attempt lasts approximately 2:30 minutes if you do it fast as possible.

Good luck with the ARG :(((

All Letters and Documents (TIPS)
First of all: ALMOST every Letter and some of the Documents have at least 3 possible locations where they can be found. The locations of the Letters/Documents are randomly generated within each 3 possible locations of each item (COD Zombies vibes)

I did not registered all possible 3 locations of each of the 30 collectibles, but I will give you some interesting tips that you can you to find all the 30 items more easily.

Before entering in specific details, I say to you: explore every room, every inch of all locations of the game.

D-Block
Most of the Letters can be found within the cells and in every single one of the three floors. When exploring a cell: look in the beds, inside the toilets (there is always a Letter inside one), on the walls and the shelves paying attention.

There are some Letters that can be found on the floor on the corridors (near the Spawn Room and in the second floor).

There is always a Letter in the Library.

Most of the times, there will be a Letter in the D-Block puzzle room.

There is a Document on the computer (fixed). Documents found on computers always have fixed locations.

Corridor 1

From what I remember, I think I had never found a Letter in this area.

C-Block

There is a chance of finding a Letter in one of the cafeteria tables on the first floor.

Almost every Letter in this area is in a cell.

When exploring a cell: look in the beds, metal plates, on the walls and the shelves paying attention.

Infirmary

There is a chance of appearing a Letter on the Main Hall and in each one of the Infirmary Rooms (on the floor).

Corridor 2

Right after unlocking the door in the Infirmary, maybe a Letter will appear in the shelve on the wall.

There is a Document (Computer) in this area.

There is a chance of appearing another document in this area.

Cafeteria

Most of times, there is a Letter that will appear in one of the cafeteria tables.

A Letter can appear on a shelve on the wall, on the stairs.

There is a Document (Computer) near the Cafeteria Puzzle machine.

Main Entrance

There is a Document (Computer) in this area.

Most of times, you can find 2 more Documents in this area. You must always try to find a total of 3 documents in this area.

There is a chance of appearing a Letter in the Interrogation Room.
