A route
Achievements:
  • Hello, Youth!
  • Life of a Mother
  • At the Park
  • New Life (can likely be unlocked through challenge mode)

Finish the previous round with no organ issues.
Issues arise when your organs change colour from having achieved a "Bad" rating.

Additional achievements:
Resurrection
If you pay to cure all organ issues, by using the 'perfect' currency (top right as you play), you'll be awarded this achievement and continue down the "A" path.

Challenge Unlocked
Once you complete A route, you'll unlock "Challenge Unlocked", which will let you play individual levels outside of story mode.
I assume this will unlock for completing B route too.

B route
Achievements:
  • Illness and Rehabilitation
  • Sweetness And Bitterness
  • Who Are You?
  • Disappearance (can be unlocked through challenge mode)

Finish the previous round with at least one organ issue that you don't pay to cure.
Organ BAD

Achievements:
  • Head is in a mess
  • I can't see!!
  • I don't feel so good..!
  • My heart is pounding
  • an organ of silence
  • I can't breathe!!

Ensure you are on NORMAL mode.
Start a run of 4A (it has more actions, but it doesn't need to be 4A).
Decide which organ you are going to let fail, and don't press that key for the duration of the attempt until it fails.
Organ MISS

Achievements:
  • In the blink of an eye
  • Miss the beat
  • Hypoxia
  • Brain stop
  • Liver Blow
  • Stomachache

Ensure you are on HARD mode.
Start a run of any level.
Decide which organ you are going to let fail, and don't press that key for the duration of the attempt.


Between this and the BAD achievements, you'll have enough to have earned the First Time and Deal With It achievements.
Instant deaths

These all relate to 'event' deaths - by pressing the button for the relevant organ when it has a flashing "!" over it, you'll die and get an achievement.

The floor is slippery
The relevant event is in 2A (and others)

Look out for car
The relevant event is in 1A (and others)

Whoosh
The relevant event is in 1A (and others)

Overdose
The relevant event is in 4B, towards the end.
Accumulated deaths

These all relate to 'accumulated' deaths - they require you to eat/drink/do something several times.

Over Eat
Keep eating food, whether good/bad for you. Pretty easy to do, and can be easily done on the 3/4 levels, as they have food that reduces hunger a lot.

Don't Starve
Don't eat food.

Can't Live without Water
Don't drink liquids.

Break through the limit
Look out for negative effects that appear on the brain. The yellow squiggles, the mail (taxes), angry co-workers. Plenty of options for you to stress yourself out with.

Smoker
This needs to be done in 3A - smoke all those filthy cancer sticks.

Heavy Eater
This needs to be done on 2B on HARD mode - there are not enough food items on NORMAL.
Eat all yellow-highlighted food items (including bubble tea).

Alcoholic
This needs to be done on 3A on HARD mode - there are not enough delicious alcoholic beverages on NORMAL.
Drink all yellow-highlighted beers/spirits. These are in cans/bottles.
Story perfects

Achievements:
  • Part1 Perfect Completion
  • Part2 A Perfect Completion
  • Part2 B Perfect Completion
  • Part3 A Perfect Completion
  • Part3 B Perfect Completion
  • Part4 A Perfect Completion
  • Part4 B Perfect Completion

Need to get good. Not much more to say - play on Hard, and get a perfect on everything.
Do all the good things in green and you'll be fine.
You'll likely need to pay attention to hunger only though, particularly in the 4s. Eating all the good food presented, 4B most notably, will slow down the beat, and give you the chance to die from overeating on good food.
Challenge mode

  • Challenge Perfect Completion
  • My hands are auto!

Sadly there is no penalty for pressing buttons for organs that don't need attention.
Since there are no 'bad' bubbled items, you can press A/S/D/J/K/L on every beat without issue.
