I made a text-based post-apocalyptic survival game a long time ago, inspired by 'The Road'. There were many ways to die due to poor circumstances or decisions, of course, but there was also a random chance to die from a heart attack, less than a 0.1% chance per turn. I only ever saw one play-tester have that happen.

"WTF, seriously?!?" he exclaimed. "Heart disease is one of the top causes of death in America, sorry". For some reason that really amused him.
