Let me know if I'm missing anything! I played this for hours and got to the point where I was never missing anything anymore, so I think I have them all - except for one, which I'll start with.

For the record, I currently have 30/40 anomalies discovered. I have no idea how to get 40/40. Might be unreleased content, might be a bug that just doesnt allow me to see some, but I played this long enough that I really can't seem to get any more unique ones. In fact, if you count the number of scenes below, you will see that there are 30, even though there are clearly more individual anomalies, so I guess it just counts scenes? My best guess, at least.

If there are separate bullet points, that means these are different anomalies from each other, and only one ever seems to show up at a time.

Giant Ice cube and fans

**Honestly, I need help with this one. I just 50/50 guess at this point, it always looks the same. I'm honestly inclined to think its bugged, but I will be happy for someone to prove me wrong and tell me what I am missing.

Guy on couch

• Parasite thing in ear
  
• Weird big toe looks like a face (the one propped on couch)

Wedding chapel

• Bride has face in the back of her head

2 Girls talking with walkers in front

• Girl on right has super creepy smile on face
  
• One legged man walks across

Ballet dancers

• Left dancer has messed up face
  
• Middle dancer has wound on back (need to wait for her to turn around)

Girl meditating in bedroom

• Meditating girl has no eyes
  
• Monster with green eyes glowing under bed

3 Knights

• One of the 3 knights will have a skull for a head

Train

• Something on the door in the back (can't quite make out what it is, a person?)

Tank

• Man points at you, which means the tank will shoot at you. You can close the door as soon as you see him point to try and avoid sanity loss / small jumpscare

Bulldozer

• Dead body in front of bulldozer. That is, directly between you and the bulldozer, not on the actual front of the bulldozer. Despite this description, surprisingly hard to see as it is charred and blends in with rock, but once you see it its hard to miss

Astronauts working on satellite

• Spider magic attack! Hard to miss...

Moose

• There are 2 small birds in the scene, either one can have a messed up head with pointy ears and a strange beak

Woman under umbrella with small pool

• Pool floatie has a very well obscured pattern. Zoom in, it moves slightly and looks like some kind of parasite but it blends in easily
  
• Woman has webbed feet

Woman, Statue, and 2 paintings (Art Gallery?)

• Statue face will look like it has a live person inside of it
  
• Left painting has a minotaur in front of a building
  
• Right painting has a man who looks like death

Samurais and Chickens

• Any of the chickens can be missing a head.

Cats

• One of the cats heads will be upside down. Can be two different cats that experience this, both are on the right side

Man in boat in cave

• Has a ghostly figure behind him, over his shoulder

Skeleton bar

• Looking at the skeleton on the left will cause it to shoot at you (totally safe, just a slight jumpscare and maybe some sanity loss?)

Graveyard

• Zombie-like head sticking out of the dirt. Can happen in front of several different graves

Big pig

• Looking at the wardrobe causes a werewolf to pop out and kill the pig

Colorful frogs

• One of many frogs may be toppled over, dead

Child's bedroom

• Elephant toy will be extremely lifelike
  
• Baby mobile toy will have a small dead man hanging from it.

TV room

• Face in one of the TVs. Can maybe happen on any of the small TVs, or at least 2 of them.

Man in library/study

• Dead person's hand sticking out from behind the chair on left
  
• Man has a tail

Hazmat suit cleaners

• One of the guys can have skull shaped indents in the back of his head

Non-descript room with open closet and boxes

• Looking at the open closet causes creepy hand to reach out and shut it closed

Big stuffed animal room

• Creepy bear sitting to the left of the big bear in the back right of the room
  
• Same creepy bear peaking out from between monkey's legs

Hunter in forest

• Monster stalking the man from right behind him and to the left slightly
  
• Same(?) monster stalking from the right of the man, only the creature's hand/claw is visible, at about knee high level with the hunter

Friends at kitchen table

• Guy in the back middle has very googly/wandering eyes, probably requires zoom to see
  
• Green melon in front of woman on the right looks like a head

Dog and 6 doghouses

• Any(or most?) of the 6 doghouses might be housing a skeleton dog
