Self Explanatory
These are the basic ones, pretty self explanatory:

First Play
Welcome.
  • Play your first game.

First Item
Collected the first item

Random Item
You tried your luck.
  • Roll the dice

Killer
Defeated 100 enemies

Surrounded
First play in surrounded mode

Demolisher
Killed at least 4 enemies with a blast
  • I believe this can be any "Blast" from a barrel, grenade, mortar, missile strike, ETC. I got this by exploding a barrel.

Death Screen
Sorry...

ELETROCUTE
Hit 4 or more enemies with a lightning bolt.
  • Get the Tesla Tower main weapon, and shoot a cluster of enemies. I do not know if the shock teleport can unlock this achievement.

Icecream
You made an enemy ice cream.
  • Freeze an enemy with a cryo mortar power-up.

Disc row
You hit 3 enemies with a disc
  • This requires the Disc main weapon which pierces enemies. just shoot a group of 3 enemies.

Super Cannon
You shoot a charged shot
  • Get the charge cannon main weapon, then hold the fire button until the energy turns red and fire it.

Super Item
Raised the level of an gadget.
  • Get the same gadget twice to level it up

CHEATER

  • Buy the hacker "Upgrade" on the second page of the terminal, If I remember correctly, you need to buy it more than once.

HP Full
Recovered 100% of HP
  • This may require rolling the full heal from the dice. I don't think healing to full is enough. I got this naturally. But is likely easiest to farm in surrounded mode, beat the first 3-4 waves and only get the dice after each wave, ensure you have taken damage or the healing options will not be on the dice.

RANK A
Received "A" rank
  • Just get a high score, you will easily get this going for the "Hero" achievement.

Stronger
Upgraded damage 3 times.

Rank S
Received "S" rank
  • Just get a high score, you will easily get this going for the "Hero" achievement.

Get off me!!
Hit 5 or more enemies with a nova blast
  • This requires the "Nova" power up, activate it in a group of enemies.

Faster
Upgraded Dash 3 times.

Survivor
Wave 10
  • Get to wave 10 in the surrounded game mode

Hero
Wave 20
  • Get to wave 20 of surrounded, this one will take an hour or two as there are more enemies for each wave, wave 19 has about 450 enemies, you get it when you reach 20, you do not need to beat wave 20.
Not So Self explanatory Ones.
These ones are for the Arena mode, just play to level 25 or so and you should get them both.

The big one
You killed a giant fly
  • A giant boss fly spawns on arena mode around level 20, a few levels after the first plane drops you some health on level 15.

Don't play with fire
Inferno Tank
  • A giant boss Inferno Tank spawns on arena mode, a few levels after the boss fly appears, I believe level 25 or so. A second boss fly might appear first, but then the inferno tank appears right after you kill it on the same level.
