Conditions to fulfill to get this achievement.

Speak with Venigni once you encounter him at the Venigni Works Control Room.
Once you reach Lorenzini Arcade, pick up the "Scandal! V, The Tragedy Behind the Flamboyance!" note. You can found it inside the bloodied bathroom in the Lorenzini Arcade. From the Stargazer, turn around and then exit out the doors with the Clemente signboard above. In the room where whimsical music is playing, turn right to find the bathroom.
When you reach Arche Abbey Outer Wall, go to the final Trinity door and talk to the King of Riddles, Arlecchino.
Read the "Scandal" book again, and talk to Arlecchino, then select the "The Truth behind the Venigni Incident" dialogue.
Afterward, talk to Venigni to get the trophy.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀THE STORY OF THE REFINED OLD LADY

Conditions to fulfill to get this achievement.

After defeating Champion Victor in the Grand Exhibition Conference Room, return to the hotel and give a gold coin to Giangio to get a cure for Antonia.
After buying the cure, talk to Polendina and select the option "Use the Cure"
After some time specifically after defeating Laxasia the Complete in Arche Abbey Prayer Room, talk to Polendina again and this will give you the achievement.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀THE STORY OF A STRANGER GIRL

Conditions to fulfill to get this achievement.

Talk to Alidoro at Relic of Trismegistus Combat Field and select the option "Attack Alidoro" twice to kill him. Pick up the item called "Alidoro's Cryptic Vessel" and bring it to Venigni so that he can decode it.
Lastly, talk to Eugénie in Hotel Krat and select the option "He was Eugénie's Older Brother".
Talk to her at a later time to obtain the trophy.

By the time you meet Sofia, you must have high level of Humanity (i.e. Your Heart is Pounding: 25+),
and for this you need to almost always . Otherwise the ending will be "Free from the puppet string".

When meeting Sophia in Arche Abbey Upper Part, choose give her peace
if you did everything right, then the following cutscenes will include your hair changing color.
When Geppetto asks to give your heart, choose the option to refuse.
Defeat the Nameless Puppet.

Before you run a NG+, head to Geppetto's Study in Hotel Krat. Get special weapon Golden Lie and read the letter that is on desk to unlock [The Story of the Blue Butterfly].

No matter what choices you make when it comes to lying, telling the truth, or even deciding Sophia's fate, you'll end up with this ending if you give P's heart to Geppetto when he asks, choose the option to give heart.

Before you run a NG+, head to Geppetto's Study in Hotel Krat. Read the letter that is on his desk to unlock [The Story of One Father].

Finish the game (any ending), the press cancel to not start NG+, you wiil spawn inside the hotel. Here, interact with the piano - you'll be able to play a perfect tune.

Upgrades*

To get this achievement all you need is to assemble handle and the blade of different normal weapons. You'll get the assembling tool after defeating Mad Donkey on Alchemist's Bridge. Gepetto will give it to you.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀ ⠀⠀STRONGEST NORMAL WEAPON

In Lies of P, upgrading Weapon Blades is a must for players who want to empower their Attack Power. This feature is accessible only after reaching Hotel Krat and speaking to Eugénie. Normal weapon Blades can be upgraded up to +10, and each level requires a certain amount of Ergo and different types of Moonstones (Hidden Moonstone to +3, Crescent Moonstone to +6, Half Moonstone to +9, and Full Moonstone for +10.) You can easily get this achievement in one playthrough.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀ ⠀ ⠀⠀STRONGEST SPECIAL WEAPON

In Lies of P, Special Weapons commonly known as Boss Weapons are unique equipment that players can get by exchanging a special type of Ergo from a defeated boss. Note that special weapons cannot be disassembled but can be upgraded using resources. Special weapons are upgraded using Dark Moon Moonstones of the Covenant and Full Moonstones of the Covenant. You can easily get this achievement in one playthrough.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀ ⠀STRONGEST LEGION ARM

As players traverse the City of Krat, they will eventually find extremely valuable parts from the chest that can eventually be crafted as new Legion Arms. These materials are called Legion Plugs, which can be acquired from chests scattered across the City of Krat while Legion Calibers are being used to modify Legion Arms. Legion Calibers drop from defeating Elite Enemies. A handsome reward for those who dare oppose these perilous enemies.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀ ⠀ ⠀EXTREME POTENTIAL

The P-Organ first becomes available after defeating the Scrapped Watchman in Krat City Hall an then speaking to Geppetto back at Hotel Krat. Afterwards, players can start purchasing abilities at the nearby red chair known as Geppetto's Tools.

Upgrades are separated into different phases.

In order to unlock a node and its skill, must fill all of a node's slots with abilities.
Slot abilities are purchased with Quartz and are immediately available after purchase.
To unlock the next Phase of Nodes, must unlock at least two Nodes in the previous Phase.
Progress to the next Phase will also unlock the corresponding Tier of Slot Abilities.

Phase 6 is unlocked by started New Game Plus while Phase 7 is unlocked by starting NG++

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀THE ULTIMATE DEFENSE TECHNIQUE

If a guard succeeds right before being hit, it triggers a Perfect Guard. When it is successful, you will not receive damage, only Stamina us consumed. You can use it to destroy an enemy's weapon or put the enemy in Staggcrable status.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀FATAL BLOW

Fatal Attacks are usually strong attack combinations that can be executed when an enemy is completely staggered. Stagger points can stack up if you continuously deals multiple hits or charge attacks to his enemies or Perfect Blocking enemy attacks on the other hand. Lastly, Fatal Attacks perform the same as backstabs.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀ ⠀ ⠀⠀BEAR GOLD COIN FRUIT

Сould be easily get after the Eldest of the Black Rabbit Brotherhood Boss. Players may use the Gold Coin Fruit in exchange for materials from Giangio, or in other cases exchange with other NPCs. Also this could be used as the material to reset P-Level, Legion arm modifications, etc.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀ ⠀ ⠀⠀FROM ACROSS THE RIFT

Have 4 types of Dimensional Butterflies, each of which giving you upgrade materials of lower or higher rarity depending on their color when killed:

Red (common)
Pink (uncommon)
Blue (rare)
Yellow (epic)

These unique creatures are usually hidden but can be revealed by Monad's Lamp, which will light up red to reveal the butterfly's location. If you happen to miss the window for defeating a Butterfly, simply rest at a stargazer, then return to the location. The butterfly will respawn in the same spot.

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀LEGION ARM COLLECTOR

Legion Arms are a special equipment for the left hand. Each arm of the Legion has its own combat ability and their own charge, can be crafted (using a Legion Plugs) and upgraded (Legion Calibers) at the Venigni Craft Machine in Hotel Krat.

Throughout your travels in the city of Krat, you will encounter NPCs and scenario where you will have to choose to tell the truth, or come up with a lie.

Rewards of the game and the ending depend on your choices.

P's Humanity is divided into levels where it shows a different message which will help you identify your Humanity Points. Level and the message that is associated with it are as follows:

Act II

Woman in the window

You’ll see the silhouette of a woman in an apartment window just before the bridge that leads to Krat City Hall. Agree to find her baby, then bring her the Broken Baby Puppet in the garden just before the Scrapped Watchman boss arena.

Lie: That’s a cute baby
Truth: That’s just a puppet

Act III (Railway in Moonlight Town)

Atoned

Lie: Give the Stalker's Greeting (you need to have a "Stalker's Promise" gesture)
Truth: Do nothing

Act IV

Alidoro, first meeting

You’ll find Alidoro once you ride the elevator past the crypts in the library area. He’ll later act as the vendor that gives you weapons and amulets in exchange for boss items.

Lie: Venigni Works
Truth: Hotel Krat

Act V

Alidoro, second meeting

If you lie to Alidoro at the first time, you can find him at Venigni Works.

Lie: Boulevard Elision (if you lie a second time, Alidoro will still come to the hotel)
Truth: Hotel Krat

Act VI

Julian the Gentleman

He’s right next to the Rosa Isabelle Street Culvert. He wants you to retrieve an item from his deceased wife. You’ll find Julian’s wife, a woman wearing a black dress, in a street corner once you get past the sewer tunnels. Pick up the wedding ring and return it to Julian.

Lie: I saw her message saying she loves you
Truth: I've never seen that happen before

Act VI

Antonia & Polendina

After defeating the King of Puppets in Estella Opera House, return to Hotel Krat and speak with Antonia.

Lie: Of course
Truth: There is nothing left

Immediately afterwards, check Polendina's counter at the front desk for the letter. Head to the garden that leads to city hall and you’ll find him next to the fountain.

Lie: Show the wedding ring (available if you lie to Julian)
Truth: There are no such puppets

Act VII

Simon Manus

After defeating Champion Victor, you'll automatically begin a conversation with Simon Manus.

Lie: Hope for a cure
Truth: Monsters

Act VIII

Eugenie

After finding Alidoro near the bridges within Barren Swamp, teleport back to Hotel Krat and speak with Eugenie who'll give you a set of gloves to deliver to Alidoro. Head back to the Barren Swamp, give Alidoro the gloves, then return to Eugenie once more.

Lie: He was happy
Truth: He was angry. He said the gloves were rubbish

Act IX

Belle

After finding Belle in the Grand Exhibition as you explore, speak with her after the earthquake and she'll ask you to find her friend in Krat Central Station. From there, go to the right and head down the stairs, turn around and you'll be able to enter an open train car. Inside the NPC will give you a letter after speaking to him. Return to Belle.

Lie: He was killed in battle
Truth: He's become a monster

Act IX

King of Riddles, telephone call

The question is not considered a riddle.

Lie: No (reward Trinity Key of The Chosen One)
Truth: Yes (reward Surprise box which will also grant the key)

If you kill Alidoro near the Relic of Trismegistus Combat Field, go to Hotel Krat and have Venigni decode his Cryptic Vessel, read the description, then speak with Eugenie.

Truth: The real Alidoro was murdered by the fake one

Head back to Venigni and he’ll give you a transmission to listen.

As you explore near the Arche Abbey Outer Wall, you'll find the last Trinity Sanctum nearby. Unlocking it with Trinity Key of The Chosen One, you'll find Arlecchino against the far wall.

When meeting Sophia in Arche Abbey Upper Part, you have to make a decision that will affect the endings.

Arlecchino's Riddles

⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀ ⠀⠀ ⠀⠀⠀⠀ ⠀⠀⠀END OF RIDDLES

You need to discover all five Trinity Sanctums due to Arlecchino's quest. Arlecchino is The King of Riddles who will challenge you by calling through telephone booths.

If you answer his riddles correctly (enhance humanity), he will reward you with Trinity Keys, or a hint that will lead you to the location of a key. These keys will give access to green doors with the Trinity mark which can give you various rewards.

Telephone Booths

Act III, Workshop Union Entrance
Found along the alleyway as you cross the bridge that leads to the factory entrance.
Act V, Malum District
Explore the level normally until you reach the open area in the slums. As you head to the back alley, you’ll spot the payphone.
Act VII, Grand Exhibition Gallery
Proceed deeper into the facility until you reach an area with two goddess statues. Head past that and you’ll spot a chamber with puppet displays. Once you’ve defeated the enemy mobs, you’ll see the telephone off to the side.

Note that this Lies of P riddle does not grant a Trinity Key instantly. Instead, you’re given a clue pertaining to the goddess statues from earlier. Return to the area and interact with the statue on the right to rotate it. This reveals a hidden panel that has the Trinity Key.
Act IX, Krat Central Station Street
The city of Krat will descend further into ruin later in the campaign. Continue until you’re well past the station and apartment. Once you reach the cliffside alleyway just before the circus arch, you’ll hear a payphone ringing.
Act X, Relic of Trismegistus Entrance
Once you’re in the cavern area, follow the winding path to reach the base of the cliff wall in the distance. There’s another payphone in the vicinity.

Trinity Sanctums

Act III, Workshop Union Culvert
Upon crossing the gap to the other side of the factory’s interior, you’ll come across a section with a large orange drain and a side path. Check the side path to find this door.
Act IV, St. Frangelico Cathedral Chapel
Climb up the tower section via the wooden beams and ladders. At the halfway point, you’ll come across a corridor with a rolling electrified boulder. Take out the zombified enemies, then look to the right of the waterwheel to spot a ladder that you can climb. Avoid the rolling boulder here, then unlock the door with a Trinity Key.
Act VI, Estella Opera House Entrance
Continue through the level normally until you reach the chamber with a wooden beam and a burning chandelier. One door leads back to the Stargazer, while the other leads to the boss’ room. However, if you look below the wooden beam, you’ll spot this door.
Act IX, Krat Central Station Lobby
As you move from the Stargazer to the open street, you’ll see this door to your left.
Act XI, Arche Abbey Broken Rift
Proceed deeper into the ancient tower, looping from the exterior to the dark chambers within. Eventually, you’ll arrive at a large room with several smaller sections connected via ladders and wooden beams. At the bottom of this area, you’ll spot the Chosen One’s Trinity Key door.

Act I, Cerasani Alley

Greatsword of Fate

Complete the Adelina's quest.

Take the right-hand stairs up from the entrance to the opera house to eventually find Adelina, who requests something sweet. You'll need to purchase the Apple for 1,000 Ergo from Polendina back at Hotel Kratand then give it to Adelina. Return to Adelina's position after beating the opera house's boss to get this record.

Act VII. From the Grand Exhibition Gallery Stargazer, turn around and go outside where there’s a Puppet soldier and a Puppet dog. Go through the shortcut to the right and climb the ladder. Climb another one and sprint to the end to find metal platforms filled with enemies. Jump down to the lower level and interact with the ladder found left to go down. Speak with Belle who’s standing in the corner to trigger her Quest.

Act VIII. Speak to her in Hotel Krat.

Act IX. Head to the Krat Central Station by the train car down the stairs to the right and speak to the NPC to retrieve the item, return to the Hotel Krat and when you report back to Belle, you need to lie to her ("He died fighting a puppet") to obtain this record.

Cryptic Vessels

You need to find and decipher all cryptic vessels and to claim hidden rewards in their locations. To decipher cryptic vessel you need to give it to Vegnini, he created this technology and he'll decipher it for you.

Act IV

Crafted Cryptic Vessel

To find Vessel you need to go from the St. Frangelico Cathedral Chapel Stargazer to the area where they have knocked the brazier off a ledge. Head down to where it dropped to see the door down the staircase is now accessible, open it to find the chest at the end of the room with book stacks and candles surrounding it. Open the chest to obtain the Vessel and give it to Vegnini.

Then, from the Krat City Hall Stargazer, head to the bridge with the hanging puppet with the note "Purge Puppets". Use a throwing object to throw onto the puppet and a obtainable item will fall to the ground. This will be the next hint.

When obtaining the Letter with the Refuge's Address, players will need to head to that building on Elysion Boulevard, in room 221b.

Act VI

Jeweled Cryptic Vessel

Starting from the Estella Opera House Entrance Stargazer, make your way up to the top floor. there will be a room at the end of the hall containing books and a ladder going up to a small area with a chest and down to an underground area. Take the ladder up to the small area with the chest that contains the Jeweled Cryptic Vessel.

Starting from the Krat Central Station Plaza Stargazer, pass the broken statue and head to the end where players will see a set of stairs going down. Take the stairs going down through an archway, and take the stairs down the cliffside. Head to the end where players will see Hotel Krat and the picture of the Jeweled Cryptic Vessel Decryption.

Go towards the dirt by the edge of the rails and use the Check Ground Gesture.

Act VIII

Old Cryptic Vessel

Starting from the Barren Swamp Bridge Stargazer, head to the cave entrance where players will see the NPC Rookie Explorer Hugo. Simply speaking to him then players will obtain the Vessel.

From the Path of the Pilgrim Stargazer, take the lift down and head down the path to find the shack up the hill. Head there to find an NPC named Slum Resident, and speak to him to obtain the Robber's Shack Key. Use the key to open the shack near him and enter to find out the floor breaks, clear the basement as there are some enemies there. Go down the hallway and take 2 ladders up, follow the path down and return to the shack.

Hug the wall the get to the other side or jump over the hole.

Act IX

Rusty Cryptic Vessel

This item can only be purchased from Polendina after giving him 3 Krat Supply Box (Standart/Sturdy/Special).

Starting from the Barren Swamp Bridge Stargazer, head into the cave where the NPC Rookie Explorer Hugo is standing by to see that it's now unlocked. Head into the cave and follow the path. Although the area seems overwhelming, the path to the next cave is quite straightforward, there are wooden planks that have a dead end, continue onto platforms that connect to each other. Players will pass the Hermit's Cave Stargazer, the cave is close by. Upon entering the cave, there is an elite enemy. Players can choose to defeat this or evade it and head to the room up the stairs where a safe can be found.

Act XI

Mechanical Cryptic Vessel

Starting from the Arche Abbey Broken Rift Stargazer, head straight into the first archway, then into the right one down the stairs. Once down the stairs, head into the doorway on the left. Clear the area of enemies, and head into the left archway again. Once players make it through the doorway, head straight into the doorway ahead. Go towards the end of the room and there will be a ladder on the right. Take the ladder up and go towards the end on the right then turn left and head onto the end to find the chest containing the Vessel. There will be several enemies in this area and a few elite enemies as well. Clearing the area will make it easier for players to get through.

Starting from the Abandoned Apartment Stargazer, head up the stairs and out the collapsed street. Take the white sign bridge across, continue along the path, and take the stairs up and cross the second white sign bridge. Head through the archway and players will be at the site of the Mechanical Cryptic Vessel Decryption image. Head up the stairs to see the Wandering Merchant. Players will need to equip the Frozen Feast Special Weapon before interacting with the Wandering Merchant.

Act X

Alidoro's Cryptic Vessel

Can be obtained after defeating the Boss Black Rabbit Brotherhood for the second time at the Relic of Trismegistus Combat Field, Alidoro will be seen by a bonfire and players will need to speak to him.

Interacting with him will give players 3 options, Purchase, Talk, and Attack Alidoro. Selecting Purchase will allow players to look through his wares and to trade in Special Weapons or Amulets if players want to. Selecting Talk will have Alidoro talk about his identity and confess his involvement in the attack on Hotel Krat. Selecting Attack Alidoro will give players another chance to continue speaking to him or to attack him. Choosing to attack will make him targetable. Hitting him once will kill him instantly. Players will get Alidoro's Cryptic Vessel upon defeating him.

To access the DLC portion, you will need to complete the base game until the end of chapter 9, after defeating the Corrupted Parade Master. From there, you will find that you have a new item in your inventory, Star's Chrysalis. With this item, you will need to fast travel to the Path of the Pilgrim Stargazer in the Malum District to watch a new cutscene. A trail will then appear for you to follow to reach a rusty Stargazer covered in vines, another cutscene will begin where P uses the Star's Chrysalis on the Stargazer to repair it. Interact with the Mysterious Stargazer and activate it to begin the DLC.

Complete the Goddard's quest.

Act I, Zoo Tram Station. Locate Goddard, a woman in a wheelchair, nearby. Speak with her, and she'll request you retrieve a "painting" near her easel, which is a blank canvas by the tram station's handrail. Pick it up, return to her, and when prompted about the painting, select: "Incredible! It is a brilliant work of art."

Act II, Elixir Reserch Zone. Next, she'll task you with finding her Art Supplies. Travel to Stargazer. From there, proceed straight through the first two open cell doors until you reach a large room. Turn right at the first opportunity, and you'll see a glowing blue item on a table beside an operating table. Collect Goddard's Art Supplies. Return to Goddard at the Zoo Tram Station and hand over the supplies.

Act IV, Archhon Fleet Shipwrecks. To complete her quest, progress through the main story until you defeat Lumacchio. Act III, Ancient Ruins Excavation Site. After crossing a wooden bridge that slopes downward, turn left into a nearby cave. Inside, you'll hear a robotic voice from a Crusher Puppet. Locate and speak with it to start the quest.

Continue through the level until you arrive at the Ergo Mining Site Stargazer. Use the moving platform to proceed, then take the right path. You'll hear the Crusher Puppet calling again. Pass the crates, follow the upward tracks, and find the puppet. Speak with it, then exit beside it to return to the main path.

Later, after exiting a cave and dropping down, turn right and cross a short wooden bridge, leading to a longer one. Traverse it to enter another cave, where the Crusher Puppet will call from the left. Speak with it again, ensuring you hear all its dialogue to advance the quest.

Act III, Pistris Harbor. From there, descend to the large platform below and follow a wooden plank path. You'll encounter a Brain in a Vat requesting assistance. Choose "Help him" when prompted to complete the quest.

Close to the end of the game, you’ll reach an area called Arche Abbey Outer Wall where you’ll be meeting a side character called Test Subject 826. After defeating Laxasia the Complete, return to this area, go down the stairs and the street and go through the shortcut gate on your left near the fire. Now continue up the bridge to find Test Subject 826. Buy the outfit and normal weapon.

Julian the Gentleman – Part 1

Talk to the man near the Stargazer. He wants you to retrieve her wife’s belongings. Accept the quest by choosing “Fetch”.

Drop down into the sewers and you’ll soon reach a fork. At the fork, go straight on and drop down into the hole below to land on an enemy. Follow the pipe to eventually find a chest. You can then use the ladder to open a shortcut back to the most recent Stargazer. Drop back down into the sewers and now at the fork take the pipe leading up and out.

When you enter the Opera House, take the right stairs. Be very careful of the wired puppets. They hit hard and affect P with disruption. If the disruption meter filles out, you die instantly. The best way to fight them is to lure them away so their wires break, becoming a lot weaker to handle. Once you’ve eliminated them, check the corner.

Continue on and approach the ringing telephone. Then, you will get a hint to solve the next part of the riddle. The Clue states “Before the fingertips of the cold woman in the Grand Exhibition”. Continue on and when you get downstairs, check your right for two statues. One of them can be moved. Move it so that it reflects the one you can’t move. A secret compartment will then open and you can collect the Trinity Key. You can now use this Trinity Key to open the Trinity Sanctum at Estella Opera House Entrance, below the swinging chandelier.

Hey, found a new item for the Overture DLC, after interacting with the brain vat, you can climb up a set of ladders right next to it that follows to another ladder that goes down into an ice area, in said ice area is a chest that contains the Storyteller's Amulet

In the Overture DLC, as soon as you enter the carnival, to your right, there's a booth where you can "Knock the clowns and receive a prize". You can actually shoot them with the bow (or throwables) and get the "Royal Flower Tea Set". Can be sold to a Wandering Mechant in the Carnival for Ergo and some lore.

One small correction, in Section IX Lorenzi Arcade, you list 2 Legion Plugs in a row, the second one is actually a Legion Caliber (dropped by the boxer)
